Oxidative stress is a key driver of metabolic health, immune function, inflammation, and hormone signaling, and it impacts nearly every system in the body.

New peer-reviewed research from our team takes a closer look at TRT monitoring, specifically comparing serum, saliva, and, to a lesser extent, urine testing in patients using transdermal testosterone.

We approached this analysis without assumptions, following the clinical data wherever it led. The conclusion is clear: serum testing should be prioritized over saliva for TRT monitoring in this context.

Here’s why:
While saliva testing often shows markedly elevated testosterone levels with transdermal delivery, these values do not correlate with clinical outcomes. Across 100+ studies, patient response, including symptom improvement and therapeutic effect, consistently aligned with serum levels, not saliva. We found no evidence that saliva values predicted clinical impact beyond what serum already indicated.

Clinical takeaway:
- Serum remains the most reliable primary tool for TRT monitoring
- DUTCH testing can serve as a valuable complement for broader hormonal insight
- Elevated saliva values with transdermal TRT should be interpreted with caution

We believe this adds important clarity to a long-standing debate and helps support more evidence-aligned decision-making in practice.

Did you know that the DUTCH Test can also be used to assess skin concerns?

Total androgen levels, androgen metabolism, the cortisol picture, and organic acid markers all tell us about the drivers for skin problems. Factors beyond DUTCH testing that influence skin health include skin barrier function, medications, genetics, food sensitivity, thyroid function, nutrient status, the microbiome, and more.

No matter the skin condition, the underlying cause, or the treatment strategy, treating skin conditions starts with addressing the skin barrier. As the skin barrier heals, triggers may make themselves more apparent, and the clinical picture becomes clearer. DUTCH testing is a useful tool for assessing contributing factors to skin conditions, even when the outcome is a normal test result that allows you to rule out the primary suspects.
