We must stop treating healthcare like an entitlement to delay chronic illness. It should be viewed as an investment into long-term wellness. Because without investing in those who provide that care, the entire system collapses. And thatโ€™s a cost none of us can afford.

P.S. This recently just hit VERY close to home as a local general surgeon in my practiceโ€™s community/hometown just ended his own life a few days ago. He left behind his wife and young family far too early BECAUSE of a broken system. Our town only has about 12,000 people in it so this happens in very big cities but it also happens in very small/rural communities.

We NEED to reverse this trend.
We NEED to find real solutions.
We NEED healthcare reform NOW.

Case Presentation

This is a 67-year-old gentleman that had a primary right hip done 10 years ago. He subsequently started developing aseptic loosening of the stem several years after surgery, but persisted on having his left hip done. I decided to cement his left hip to avoid distal potting and proximal loosening like he had on the right side.

He was able to last for about eight years before finally deciding to get his right hip revised.

I revised it through a Direct Anterior (DA) approach using a short, anatomical, side-specific, metaphyseal-filling stem to revise it as opposed to a longer more diaphyseal-engaging stem (Medacta International MiniMax).

I have used this stem in the past for primary THAโ€™s about a dozen times or so, and liked the concept, but had a handful of intraoperative calcar fractures when implanting the stem. I decided to place a prophylactic cable in this revision scenario to help avoid that.
