Becki Lentz Physical Therapy & Fitness Center Fitness center offers Matrix strength & cardiovascular equipment and a free weight room. We look forward to working with you!

Our team of licensed physical therapists will work in conjunction with your referring provider to evaluate and treat what ails you. Your doctor can simply fax us a referral and we will contact you for scheduling (fax # 231-972-7981). We accept a wide range of insurance plans, including Medicare, BCBS, Priority Health, Auto and Workman's Compensation Claims. Please be sure you are aware of your plan benefits as you will be billed separately for any costs related to your care. We also have a public Fitness Center which offers Matrix strength & cardiovascular equipment and a weight room. We offer various membership plans for you to choose from and are also an approved facility for programs like Silver Sneakers and Renew Active! FITNESS CENTER HOURS:

· Monday - Thursday: 7:00 am - 5:00 pm
· Friday: 7:00 am - 3:00 pm
· Saturday: 7:00 am - 1:00 pm
· Sunday: Closed

MEMBERSHIP PLAN OPTIONS (Requires a ONE TIME ENROLLMENT FEE of $35.00* and a recurring rate as listed below):

• Monthly = $30.00
• Six Months = $150.00 (buy 5 months, get 1 free)
• Yearly = $300.00 (buy 10 months, get 2 free)

*Includes fitness analysis, general fitness program, and one on one orientation to all strength and cardiovascular equipment. PAY PER VISIT OPTIONS (No enrollment fee required):

• 10 visits = $40.00
• 20 visits = $60.00
• 1 day visit = $5.00
