Does your loved one struggle with their medications? Maybe sometimes you find a pill lying on the counter and they are unsure where it came from. Or do you notice that they are suddenly having a lot of blood pressure problems or needing to be seen frequently? These can all be signs and symptoms of medication issues or signs and symptoms that medications are not being taken correctly. Medication management and the misuse of medications is the number one reason why our seniors are hospitalized. Client Care Coordinator / Position Filled
Pay: $18.00 - $26.00 per hour

Description:
This role is solely based on the admission of new clients into service, instructions and care coordinated actions related to the caregiver and the client. The primary responsibility for this role is to manage the consult and client onboarding pipeline. The case management specialist is responsible for maintaining relationships with our community partners and our caregivers.
Duties:
Main point of contact for consults and onboarding new clients by providing accurate documentation in accordance with OHA standards.
Participate in complete communication and warm hand-off of all new clients to care staff ensuring care plans signed by caregivers.
Maintain client charts to ensure activation and deactivation done in in a timely manner – same day.
Community Partnerships Liaison (network at events, gifts, manage marketing relationships)
Caregiver training / Coaching / Mentoring - Maintain training recommendations and care aspects with care staff – back up for Orientation and Skills Training.
Monitor all documentation of care team members on our EHR weekly dashboard
Maintain inventory of all equipment list for care needs such as DME, lockbox, medication administration records, monitors, cameras, chair and bed alarms
Maintain client charts with OHA compliance for initial monitoring visits within 7 to 30 days, supervisory visits every 90 days.
Update care plans based on client charts. Triage and report to RN / Agency admin care concerns.

QUALIFICATIONS:
Candidates must have at least 2+ years of experience in caregiving/patient care or skilled nursing care work such as CNA.
2+ years of experience in Medical Industry as either a CCMA, CNA, EMT, or LPN preferred
Bachelor's degree in a related field (nursing, biology, etc.) is preferred
3+ years of experience hands on patient care in any clinical setting is required
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Willingness to travel up to 50% of the time
Proactive self-starter
Professional, assertive, and clear verbal and written communication skills
Proficient in Microsoft Office applications (Word, Excel, Outlook, PowerPoint, OneDrive, Teams)
COMPENSATION AND BENEFITS:
This is a full-time position paid hourly based on experience.
This administrative position is eligible for medical, vision, dental after 30 days. PTO and 401K are available after 90 days employment.
Employee discounts are available at our Clinic services.
Office Hours are Monday-Friday 9:00-5:00pm, with an expectation of on-call admin phone accessibility in accordance with rotation of all admin staff. On-call is an additional flat fee stipend of $15 weeknight from 5pm- 8am the next morning and a stipend of $150 per weekend rotation.

ESEENTIAL DUTIES AND RESPONSIBILITIES:

Manages image library and sourcing online stock photography for brands
Audits Marketing OneDrive/SharePoint including file organization structure for team efficiency and productivity

Involved in payment approval process and works with accounting

QUALIFICATIONS
2+ years executive assistant / personal assistant experience
2+ years of traditional marketing experience
Hospitality and event planning experience preferred
Excellent organization and time management skills

Ability to manage projects across multiple departments
Strong business acumen and strategic thinking
Proactive self starter
Professional, assertive, and clear verbal and written communication skills

Office Hours are Monday-Friday 8:30-5:00pm, with an expectation of on-call admin phone accessibility in accordance with rotation of all admin staff. On-call is an additional flat fee stipend of $15 weeknight from 5pm- 8am the next morning and a stipend of $150 per weekend rotation.
This administrative position is eligible for vision, dental, as well as ability to select disability insurance, life insurance, wellness and cancer coverages as a supplemental insurance after 30 days. PTO is accrued and eligible to be used by employee after 90 days of employment. A Savings program is also available for participation as well as 401K participation after 6 months of employment.
