Jackson Care Connect Jackson Care Connect provides physical, behavioral and dental health care for Oregon Health Plan (Medicaid) members in Jackson County, Oregon. to 5 p.m.

Jackson Care Connect empowers people and systems to improve the health of our community. In Jackson County, OR, CareOregon and Jackson County Mental Health joined together to serve Oregon Health Plan members as Jackson Care Connect. Jackson Care Connect works with the state of Oregon to provide health services to members of Oregon Health Plan (OHP), the state's Medicaid plan. With the help of members, providers, hospitals and communities, Oregon has created a new type of health plan called a Coordinated Care Organization (CCO). Jackson Care Connect is a CCO, where you can get all of your health care from the same plan, resulting in better, more effective care. This means that you receive physical, mental and dental health services from Jackson Care Connect. Provider groups, hospitals, health plans, mental health organizations and agencies in Jackson County have joined together to serve you through Jackson Care Connect. Access to quality care shouldn’t depend on where you live. In Jackson County, a powerful partnership is helping make that a reality.

We have teamed up with Mercy Flights Inc on two innovative pilot programs that help people get the right care, at the right time.

Through Mobile Integrated Healthcare (MIH) and the Alternate Destination pilot, Mercy Flights paramedics and our coordinated care teams are reducing repeat 911 calls, easing ER overcrowding and connecting patients to the follow-up care they need.

CareOregon is proud to support this partnership, which helps improve outcomes for Oregon Health Plan members and strengthen the safety net for Southern Oregon communities.

The RVTD Stuff the Bus: Food Drive kicks off tomorrow! All food collected will go toward the ACCESS Food Assistance Program, which distributes groceries to community members through a network of local food pantries and meal programs.

The Oregon Department of Human Services (ODHS) has announced that full November SNAP benefits have been loaded onto EBT cards.

Before heading to the store, ODHS encourages checking your EBT balance prior by using the official EBT Edge website or mobile app.

We are proud to support community members impacted by the recent SNAP food benefit disruption by donating $10,000 to United Way of Jackson County's Food is Fundamental campaign, which is helping local food banks meet rising demand across Southern Oregon.

“JCC is so grateful for community partners like United Way who are moving quickly to help community members who are affected by SNAP disruptions,” says Sam Watson, Director of Community Health Partnerships for JCC. “Being able to put food on the table is essential to our members’ physical and mental well-being.”

A huge congratulations to our partners Mercy Flights Inc and the MIH Team for this national recognition! 💚

We’re proud to share that our Mercy Flights Inc. MIH Team received the National Association of Mobile Integrated Healthcare Providers (NAMIHP) MIH Champion Award for Team Approach and Interprofessional Care at EMS World in Indianapolis this past week, a huge national recognition for our southern Oregon team!

Serving over 225,000 residents across nearly 2,800 square miles, the MIH program brings together emergency medical services, healthcare, and social services to meet patients where they are. In partnership with Asante, Providence Medford Medical Center, Jackson Care Connect, Allcare, UNETE, ACCESS, and Columbia Care, the team provides urgent and long-term support to those most at risk.

Due to the federal government shutdown, people who get SNAP food benefits in Oregon may not get their November food benefits on time. We understand this news may feel stressful. Food benefits are important and delays are hard.

The Oregon Department of Human Services (ODHS) is working to keep people informed and will continue to share updates.

ODHS encourages SNAP participants to:
• Check your EBT card balance regularly.
• Continue following SNAP rules and reporting requirements.
• Stay informed by following or subscribing to ODHS communication channels.
• Sign-up for a ONE Online account and download the Oregon ONE Mobile app to get notices about your SNAP benefits. (This is the fastest way to get notifications about SNAP benefits.)

To find emergency food resources in your community:
• Visit the ODHS website for food support
• Visit the Oregon Food Bank website to find food banks near you
• Call or text 211
• Older adults and people with disabilities can connect with the Aging and Disability Resource Connection of Oregon (ADRC)
