Jackson Care Connect Jackson Care Connect provides physical, behavioral and dental health care for Oregon Health Plan (Medicaid) members in Jackson County, Oregon. to 5 p.m.

Jackson Care Connect empowers people and systems to improve the health of our community. In Jackson County, OR, CareOregon and Jackson County Mental Health joined together to serve Oregon Health Plan members as Jackson Care Connect. Jackson Care Connect works with the state of Oregon to provide health services to members of Oregon Health Plan (OHP), the state's Medicaid plan. With the help of members, providers, hospitals and communities, Oregon has created a new type of health plan called a Coordinated Care Organization (CCO). Jackson Care Connect is a CCO, where you can get all of your health care from the same plan, resulting in better, more effective care. This means that you receive physical, mental and dental health services from Jackson Care Connect. Provider groups, hospitals, health plans, mental health organizations and agencies in Jackson County have joined together to serve you through Jackson Care Connect. To ensure you get the best care, we’ll coordinate your care with all of the providers or facilities you use. We are happy to announce the return of the Fill the Backpack initiative at all Medical Eye Center Locations. We are asking that all patients (or anyone that wants to participate) with an appointment this month bring with them some new school supplies to add to the backpacks we will be giving away at the Healthy Start Block Party Saturday, August 23rd at Phoenix High School. Summer is bringing hot temperatures this week to the Portland metro region. National Weather Service - Portland has issued an Extreme Heat Watch in effect Tuesday, July 15 through Thursday, July 17.

Prolonged heat can increase the risk of heat-related illnesses, primarily for individuals extremely sensitive to heat without access to shade, cooling or hydration.

Tips to help limit heat risk:
β€’ Limit strenuous outdoor activities, especially during the hottest times of the day
β€’ Drink plenty of water and fluids to stay hydrated
β€’ Avoid direct sun and find shade whenever possible
β€’ Use a wet towel or cooling cloth to lower body temperature
β€’ Visit local cooling centers
