Jackson Care Connect

Jackson Care Connect Jackson Care Connect provides physical, behavioral and dental health care for Oregon Health Plan (Medicaid) members in Jackson County, Oregon. to 5 p.m.

Jackson Care Connect empowers people and systems to improve the health of our community. In Jackson County, OR, CareOregon and Jackson County Mental Health joined together to serve Oregon Health Plan members as Jackson Care Connect. Jackson Care Connect works with the state of Oregon to provide health services to members of Oregon Health Plan (OHP), the state's Medicaid plan. With the help of members, providers, hospitals and communities, Oregon has created a new type of health plan called a Coordinated Care Organization (CCO). Jackson Care Connect is a CCO, where you can get all of your health care from the same plan, resulting in better, more effective care. This means that you receive physical, mental and dental health services from Jackson Care Connect. Provider groups, hospitals, health plans, mental health organizations and agencies in Jackson County have joined together to serve you through Jackson Care Connect. To ensure you get the best care, we’ll coordinate your care with all of the providers or facilities you use. For more information, go to www.jacksoncareconnect.org or call 1‐855‐722‐8208, 8 a.m. Mondays through Fridays.

Colorectal cancer can affect anyone — but you’re at higher risk if you’re 45 or older, have a family history, have inflammatory bowel disease or have certain lifestyle factors.

Even if you don’t have these risk factors, it’s important to get routine screenings (starting at age 45), as colorectal cancer can often develop with no initial symptoms.

Know the signs:
• Abnormal changes in bowel habits
• Blood in stool
• Persistent abdominal pain, bloating or cramping
• Unexplained weight loss or fatigue

Early detection saves lives. Get screened for colorectal cancer – it's covered for CareOregon members. To learn more visit, https://ow.ly/JWsH50YqoU6

Myth: Healthy people don’t need the flu vaccine.
A common flu vaccine myth is that only high-risk individuals such as the elderly, young children and those with chronic conditions need the flu vaccine.

Fact: Everyone, regardless of age or health status, can benefit from the flu vaccine.
The flu vaccine helps protect everyone from illness and reduces the spread to people who may be more vulnerable.

Flu shots are FREE for CareOregon members. To learn more and access resources visit, https://ow.ly/4Gry50YpPBN

March is Colorectal Cancer Awareness Month, a time to highlight the importance of early detection and screening.

Colorectal cancer begins in the colon or re**um. While most people are diagnosed around age 68, rates are rising in younger adults: 1 in 5 people diagnosed with colorectal cancer are now under the age of 55 – making screening more important than ever.

Early detection saves lives. Colorectal cancer is highly treatable when caught early.

Healthy smiles start early! 😄 Building good dental habits at home can help protect your child’s teeth for years to come. Here are a few simple tips to get started:

• Gently clean your baby’s gums with a soft cloth and begin brushing daily after the first tooth comes through.
• For kids under the age of 3, brush with a rice sized amount of fluoride toothpaste.
• For kids over the age of 3, brush with a pea sized amount of fluoride toothpaste.
• Limit sugary drinks like juice and soda.
• Support children with brushing and flossing until around age 8.

Small habits today can make a big difference for lifelong oral health. 🦷

Access to quality care shouldn’t depend on where you live. In Jackson County, a powerful partnership is helping make that a reality.

We have teamed up with Mercy Flights Inc on two innovative pilot programs that help people get the right care, at the right time.

Through Mobile Integrated Healthcare (MIH) and the Alternate Destination pilot, Mercy Flights paramedics and our coordinated care teams are reducing repeat 911 calls, easing ER overcrowding and connecting patients to the follow-up care they need.

CareOregon is proud to support this partnership, which helps improve outcomes for Oregon Health Plan members and strengthen the safety net for Southern Oregon communities.
