Jackson Care Connect Jackson Care Connect provides physical, behavioral and dental health care for Oregon Health Plan (Medicaid) members in Jackson County, Oregon. to 5 p.m.

Jackson Care Connect empowers people and systems to improve the health of our community. In Jackson County, OR, CareOregon and Jackson County Mental Health joined together to serve Oregon Health Plan members as Jackson Care Connect. Jackson Care Connect works with the state of Oregon to provide health services to members of Oregon Health Plan (OHP), the state's Medicaid plan. With the help of members, providers, hospitals and communities, Oregon has created a new type of health plan called a Coordinated Care Organization (CCO). Jackson Care Connect is a CCO, where you can get all of your health care from the same plan, resulting in better, more effective care. This means that you receive physical, mental and dental health services from Jackson Care Connect. Provider groups, hospitals, health plans, mental health organizations and agencies in Jackson County have joined together to serve you through Jackson Care Connect. To ensure you get the best care, we’ll coordinate your care with all of the providers or facilities you use. Yesterday we celebrated the ribbon-cutting of AGE+'s Oro Vista Village in Talent, Oregon — 26 affordable, accessible homes for older adults, along with a vibrant community space that fosters connection and well-being. Jackson Care Connect is proud to have supported this inspiring project that helps build stronger, more inclusive communities for aging with dignity.

As people live longer, AGE+ is leading the way with innovative, intergenerational strategies that address housing, mobility, and caregiving gaps.

Flu season has started; it's time to get vaccinated! Getting the flu shot is a safe, easy way to protect ourselves and our communities from severe illness and reduce the chances of a trip to the emergency room.

💉 Who should get the flu vaccine?
Everyone 6 months and older, with rare exception, should get vaccinated every flu season. It’s especially important for people with chronic underlying health conditions and risk factors.

October is Breast Cancer Awareness Month! 🩷

Breast cancer is one of the most common cancers among people assigned female at birth, though it can affect anyone. Breast cancer is a disease characterized by uncontrolled cell growth in the breast. These cancer cells can form a lump, called a tumor and may spread to other parts of the body if not treated.

A mammogram is the best way to catch breast cancer early. Talk to your doctor about getting screened, early detection saves lives!

Exciting news for Rogue River families! The Rogue River School District has officially opened its new Early Learning and Wellness Center. This facility will provide children and families with Head Start early learning opportunities, childcare and access to primary health care.

We were proud to provide start-up funding to support La Clinica, who will operate the on-site health and wellness clinic. Thank you, La Clinica, for helping meet this critical need for students and families.

The Early Learning and Wellness Center represents an important step forward — supporting the growth, health and overall well-being of the community.
