At ColumbiaCare, we believe it's important to share client stories as a way to celebrate the important work happening at each of our programs.

ColumbiaCare began serving an individual through our Intensive Case Management (ICM) program after a stay at the Behavioral Health Unit, and while they were experiencing houselessness and substance use. There were ongoing challenges achieving consistent engagement with the client due to a pattern of hospitalization, crisis center stays, and returns to houselessness--which also resulted in returns to substance use. During their touchpoints with the client, the team was able to identify key indicators that the person truly needed a higher level of care. They began the process of getting them connected to Developmental Disability Services (DDS), and after multiple meetings and collaborative efforts, the client was successfully connected to an adult foster home in Portland. The client is now being connected to all the necessary and helpful supports in the new area. The collaboration between the ICM team, our Psychiatric Services Team, the Beckett Center staff, and Jackson County DDS services has contributed greatly to this successful transition.
