Our team provides medical, emotional, and spiritual care to patients with life-limiting medical conditions. Death does not have to be imminent to qualify for hospice care. Examples of qualifying medical diagnoses include cancer, Alzheimer's, heart disease, lung disease, kidney disease, Parkinson's disease, ALS, AIDS, and others. The hospice team includes physicians, nurses, aides, chaplain, social worker, and volunteers. Bereavement care is provided to families of patients for a year after the death.
