✨ Now Offering Support for Families Caring for Loved Ones with Dementia ✨

Caring for a loved one with dementia can be rewarding—but it can also be overwhelming. If you are an unpaid family caregiver, there is help available through Medicare.

Through Medicare’s GUIDE Program, eligible individuals with Medicare Part A & B and a diagnosis of dementia may qualify for up to 80 hours of home care services per year—at NO COST.

💙 What this means for you:

Relief and support for unpaid family caregivers
Assistance with daily care needs in the home
Time for you to rest, recharge, or handle personal responsibilities

💡 Eligibility Requirements:

Must have Medicare Part A & Part B
Must have a diagnosis of dementia
Must have an unpaid family caregiver providing support

✅ Already receiving services through a waiver? You may still qualify!
Individuals on the Pathways for Aging Waiver or Health & Wellness Waiver may still be eligible.

➡️ Important: These services do NOT replace your current approved hours—they are additional hours of care provided to support you and your caregiver.

✔️ No out-of-pocket cost
✔️ Covered by Medicare
