Strong Bones, Strong Muscles, Strong Women

When we think about osteoporosis prevention, most people immediately think of calcium supplements.

Calcium is important—but bone health is only part of the picture.

Bone strength depends on a whole system working together:
hormones, muscle mass, nutrient intake, and metabolic health.

After menopause, declining estrogen and testosterone can accelerate both bone loss and muscle loss. That combination is one reason falls and fractures increase with age. In fact, loss of muscle strength is one of the strongest predictors of loss of independence later in life.

Emerging research is exploring how restoring physiologic hormone levels may help maintain strength and function. In a recent study of women recovering from hip fractures, participants receiving exercise plus testosterone therapy were nearly twice as likely to stop relying on walkers or canes compared with those receiving placebo.

What’s important to note is that the doses used were physiologic—within normal female ranges—and did not increase serious adverse events.

Research like this is helping expand the conversation about women’s health beyond the traditional focus on bone density alone.

Important update we’re seeing right now:

The commercial product Prometrium (oral micronized progesterone) has been intermittently on backorder. Some patient can’t tolerate commercial progesterone due to peanut allergies and/or fixed capsule dosages of 100mg and 200mg capsules.

That can create real disruption—especially for women who are finally sleeping better.

Not All Supplements — or Advice — Are the Same

Walk into almost any store today and you’ll see shelves full of supplements promising better sleep, more energy, stronger immunity, or improved performance.

But here’s something many people don’t realize:

The right supplement taken the wrong way may not work at all.

At Hobbs Pharmacy, one of the most common conversations we have with patients is about how to take supplements correctly — not just which ones to take.

For example, someone might say magnesium didn’t help their sleep — only to discover they were taking it at breakfast instead of before bed. Another patient might say probiotics “didn’t work,” when they were being taken at the same time as an antibiotic that was eliminating them before they could help.

We also see supplements like CoQ10 taken on an empty stomach, even though the body absorbs it far better when it’s taken with a meal containing healthy fats. Small details like this can make a big difference in whether the supplement actually does what it’s supposed to do.

Quality matters too.

Unlike prescription medications, supplements are not regulated in exactly the same way. Products can vary widely in purity, dosing accuracy, bioavailability, and third-party testing. Some may contain far less of the active ingredient than the label suggests, while others may include unnecessary fillers or poorly absorbed forms of nutrients.

What Exactly Is Hormone Therapy?

As women move through perimenopause and menopause, many begin to hear the term hormone therapy (HT)—sometimes still called hormone replacement therapy (HRT). But what does it actually mean?

Hormone therapy refers to the medical use of hormones to restore levels that naturally decline during midlife. Most commonly, therapy may involve estrogen, progesterone, and in some cases testosterone or DHEA. The goal is not to “reverse aging” or push hormone levels beyond normal ranges. Instead, it’s about supporting the body when natural hormone production begins to decline, helping restore balance in systems that rely on those hormones.

Hormones affect far more than reproduction. They influence bone health, sleep, brain function, skin integrity, metabolism, muscle maintenance, and cardiovascular health. When hormone levels shift, many women notice changes in energy, sleep quality, mood, skin, and overall well-being.

Hormone therapy can be delivered in several different forms depending on individual needs. Some therapies are transdermal, such as creams, gels, or patches applied to the skin. Others may be localized treatments used in the vulvar or vaginal area to support tissue health. Certain medications, such as micronized progesterone, are often taken orally. The right formulation, dose, and route of administration vary from person to person.

Is Hormone Therapy the Same as Birth Control?

No. Birth control pills work by suppressing the body’s natural hormone production in order to prevent ovulation.

Hormone therapy does the opposite. It is designed to replace hormones that the body is no longer producing in sufficient amounts, helping support normal physiologic function during menopause.

What About “Bioidentical” Hormones?

The term bioidentical simply means that the hormone has the same molecular structure as the hormones naturally produced by the body. Many FDA-approved medications already use bioidentical forms of estrogen and progesterone.

Compounded hormone preparations can sometimes be helpful when patients need customized doses, specific formulations, or alternative delivery methods that are not commercially available. However, compounded hormones are not automatically safer, stronger, or more “natural.” What matters most is appropriate dosing, the correct formulation, and individualized medical guidance.

The Bottom Line

Hormone therapy is one tool among many that may help support women during perimenopause and menopause. For some women it can be incredibly helpful, while for others different approaches may be more appropriate.

Skin Aging Isn’t Just a Skincare Issue — It’s a Hormone Story Too

For years, we’ve talked about aging skin as if it were only about skincare products—serums, moisturizers, and actives.

But biologically, skin is more than cosmetic tissue.
Skin is hormone-responsive tissue.

Estrogen receptors are found throughout the skin, both in the epidermis and dermis. Estrogen influences several key factors that help skin stay healthy and resilient, including collagen production, hydration, elasticity, wound healing, and the strength of the skin barrier.

This is one reason many women notice significant skin changes during perimenopause and menopause. Research suggests that collagen levels can decline as much as 30% in the first five years after menopause, contributing to thinner, drier, and more fragile skin.

Recently, some clinicians and researchers have been discussing the off-label use of low-dose estrogen creams—such as estriol or estradiol—applied to facial skin as a potential way to support skin structure.

To be clear, this approach is considered off-label, meaning it is not specifically approved by the FDA for facial skin use. Long-term randomized trials on facial application are still limited, even though we have decades of research showing how estrogen supports skin health overall.

Because hormones affect many systems in the body, this type of therapy is not appropriate for everyone. It should always involve a discussion with a healthcare provider who understands your health history, hormone status, and personal goals.

🦴Bone Health Isn’t Just About Calcium

Most people know calcium matters for bone health. But what many women don’t realize is that bone strength is deeply connected to hormones and muscle health too.

Here’s a statistic that stops many people in their tracks:
1 in 2 women will experience an osteoporotic fracture in their lifetime.

Hip fractures are particularly concerning—not just because of the injury itself, but because of what often follows. About 1 in 5 women die within a year after a hip fracture, and many others never regain their previous mobility or independence. Walkers, canes, and assisted care can become permanent parts of life after the injury.

So researchers have begun asking an important question:
What helps women regain strength and independence after these fractures?

A study published in JAMA Network Open looked at women over age 65 recovering from hip fractures. Participants were assigned to exercise with testosterone therapy, exercise with placebo, or usual care.

After six months, walking distances were similar across groups—but something fascinating emerged. Nearly twice as many women using physiologic testosterone therapy were able to stop relying on walkers or canes compared with placebo.

Why might that happen?

Because testosterone—yes, even in women—plays a role in:
• supporting muscle protein synthesis
• rebuilding lean muscle after injury or illness
• improving strength, balance, and stamina
• supporting neuromuscular recovery

After menopause, both estrogen and testosterone decline, which contributes to faster muscle loss and weaker bones. That combination increases the risk of falls, fractures, and loss of independence over time.

💊Timing Matters — When You Take a Supplement Can Change the Outcome

At Hobbs Pharmacy, we often see patients who are doing the right thing — trying to support their health with supplements — but sometimes the timing makes all the difference.

A supplement that helps one person feel better can sometimes make another feel worse simply because it’s taken at the wrong time of day.

Take magnesium, for example. Many people take magnesium glycinate in the morning thinking it will help with muscle tension or stress. But glycinate tends to have a calming effect on the nervous system and can support healthy sleep architecture. Taking it early in the day may waste that benefit. For many people, moving magnesium glycinate to 60–90 minutes before bedtime helps support deeper rest. If magnesium is needed during the day for muscle function or energy metabolism, forms like magnesium malate may be a better daytime option.

Another common mistake involves B vitamins. These vitamins play an important role in energy metabolism and neurotransmitter production. Because they can be stimulating, taking a B-complex late in the day can sometimes make it harder to fall asleep. Most people do best taking B vitamins in the morning with breakfast when the body is naturally ramping up energy production.

Timing can also matter when managing histamine-related symptoms. Some patients only take antihistamines when symptoms flare, but for individuals dealing with mast cell–related issues, reactive dosing often allows histamine levels to rise before medication has a chance to help. In some cases, scheduled dosing recommended by a healthcare provider helps maintain a steadier baseline.

Even nutrients like CoQ10 can be affected by timing. CoQ10 is fat-soluble, meaning the body absorbs it much better when taken with food that contains healthy fats. Taking it on an empty stomach can significantly reduce how much your body actually absorbs.

And when someone is prescribed an antibiotic, probiotics can still be helpful — but they shouldn’t be taken at the exact same time. Antibiotics can kill the probiotic organisms before they have a chance to support the gut microbiome. Spacing them at least two hours apart gives probiotics a better chance to do their job.

The takeaway?

Small adjustments in timing, form, and dosing can make a meaningful difference in whether a supplement truly supports your health.

Why so many women wake up at 3 AM — and why it’s not “just stress.”

A woman came into the pharmacy recently and said something I hear more often than you might think.

“I fall asleep fine… but every night around 3 AM I’m wide awake. My brain turns on, and I can’t get back to sleep.”

She assumed it was stress.
Or too much caffeine.
Or just part of getting older.

But when we talked a little more, a pattern appeared.

She was in her early 40s.
Her cycles had become less predictable.
And the sleep disruption had slowly crept in over the past year.

What many women aren’t told is that sleep is often one of the earliest places hormonal changes show up.

During the perimenopause transition, ovulation becomes less consistent. One of the first hormones affected is progesterone—a hormone that quietly acts as one of the body’s natural calming signals. Progesterone interacts with GABA receptors in the brain, the same pathways targeted by many prescription sleep medications. When progesterone drops, sleep can become lighter, more fragmented, and those early-morning wakeups start appearing.

At the same time, hormone shifts can influence cortisol rhythms, leading to a surge in the early morning hours. That’s why many women describe the same experience: waking between 2–4 AM with a racing mind, even when they were exhausted just hours earlier.

Add in other changes—like night sweats, hot flashes, or stress sensitivity—and deep restorative sleep becomes harder to maintain.

The result isn’t just feeling tired. Poor sleep affects metabolism, mood, memory, recovery, and long-term health.

The good news is there are solutions, but they require understanding the root cause.

For some women, addressing hormonal changes with a healthcare provider may be appropriate. Others benefit from evidence-based strategies like cognitive behavioral therapy for insomnia (CBT-I), targeted nutrition, strength training, or specific nutrients that support the brain’s calming pathways.

But the most important step is this: realizing you’re not imagining it, and you’re not alone.
