Our core team includes home-visiting physicians, skilled hospice nurses, clinical social workers, chaplains, hospice aides, specially-trained volunteers, and grief counselors. We also offer physical therapy, occupational therapy, speech therapy, dietary consultation, and complementary therapies such as pet, massage, and music therapy. A hospice consultation to discuss the philosophy, criteria, and services of Americare Hospice & Palliative Care is done to inform the patient and caregivers and see if hospice is right for them. The patient’s physician gives confirmation of the patient’s life-limiting illness. Once enrolled under hospice, a nurse will come to do the admission visit and initiate the plan of care with the patient, family, and team.
