Oakwood Creative Care’s Hub, now open in Mesa and Chandler, is Arizona’s first-ever wrap-around model supporting the aging journey through every stage of life. The Hub connects individuals living with dementia, their care partners, and even neighbors in the community with expert-led resources, education, and wellness opportunities designed to strengthen both brain and body health.
