Every person we serve has a story, and sometimes we get to help make the final chapter a little brighter.

Recently, our hospice team at Advanced Home Health and Hospice of Arizona came together to create a “perfect day” for one of our patients — A lifelong Chicago White Sox fan, Clarence wanted to see his team one more time. The team rallied and made it possible for Clarence and his wife to attend a spring training game.

It's was a reminder that hospice isn’t just about medical care, it’s about celebrating live and creating memories.

We’re grateful to our caregivers, partners, and this remarkable family for allowing us to be part of such a meaningful day.

Our patient had been bedbound for quite some time and was understandably nervous about using her Hoyer lift. With gentle encouragement and support from her caregiver and our medical team, she decided to give it a try so she could enjoy some fresh air.

She absolutely loved it. 🌞

Now, as she’s able, she looks forward to getting out of bed and spending time outside. Moments like these remind us that home care is about comfort, dignity, and helping patients experience meaningful moments — no matter how big or small.

We’re grateful for the moments that bring joy and connection. One of our hospice patients was especially excited to enjoy the big championship football game this past weekend surrounded by loved ones. Our hospice nurse wanted to make sure he was game day ready, with a shirt to rep his favorite team 🏈

Occasions like this remind us of the meaningful impact that comfort, community, and togetherness have. These are the memories that truly matter. 💛
