Every day is different for an eye care professional, patient care can range from routine care to an emergency referral to specialists. Optometrists play a crucial role in managing diabetes, a leading and growing reason for vision loss. Diabetic retinopathy is divided into two categories, proliferative and non-proliferative. Treatment protocol varies depending on severity. For some monitoring with photos, oct and dilated exams every six months or yearly is recommended, but others are referred to specialists for injections, laser treatment or surgery.

Duration of the disease and Glycemic control are two aspects that put patients at risk for diabetic retinopathy. Retinopathy presents within the retina in form of hemorrhages, cotton wool spots (ischemic areas), exudates (leakage of lipids from capillaries), neovascularization (new abnormal vessels), macular edema (swelling within the best vision area) and other complications. Duration of greater than 5 years predisposes the patient to some type of retinopathy. Glycemic control is measured by a lab test HbA1c (measurement of sugar to hemoglobin) that gives a three-month average. In general, a HbA1c of 7% or less is recommended. For 1% decrease in Hb1Ac, there is a 35 % risk reduction in retinopathy.

Early detection and prompt treatment of diabetic retinopathy is 90% successful in preventing severe vision loss. Patients should be educated by their primary care physicians and endocrinologists to get a yearly dilated retinal exam.
