General information

Every patient gets their own Hospice Treatment Team that works together to ensure quality of care and cohesiveness. Our Team includes: *Medical Director / Attending Physician, who leads interdisciplinary team in development of the Plan of Care, Provides consultation to MD’s regarding Hospice care, and Certifies terminal prognosis. *Nursing Services, which provide Medication and symptom management, Wound Care, Caregiver Education, ADL Assistance, On Call 24/7 *Medications, Treatments, DME, Supplies (Everything needed to manage symptom control for the hospice diagnosis) *Social Work Services, which provide Emotional and Counseling Support for the patient and family, as well as Community Resource assistance *Bereavement Coordinator, who acts as Spiritual Support for the patient and family, Continues family service for minimum 13 months following patient death *Volunteer Coordinator, who Provides assistance as available to “Take the load off” primary caregivers
