EM Academy Dr Moussa obtained his Bachelors degree in dentistry from Alexandria University, Egypt in 2009.

Our mission is to aid clinicians on their journey to master regenerative and dental implant surgery through innovative, generous content that’s presented in a clear step-by-step approach. He then joined his father in a specialty practice with a focus on dental implant rehabilitation. In 2012 Dr Moussa started a fellowship program in periodontics at Case Western Reserve University (CWRU) in Cleveland, Ohio. He then persued a Masters degree in periodontics and joined the residency program at CWRU. Recognizing the importance of a sound understanding of prosthodontics in a surgical practice, Dr Moussa joined the fellowship program at Louisiana State University in New Orleans, Louisiana. He currently maintains a full time position at LSU with focus on complex restorative dental implant treatment. He also maintains a part time private practice position at The Perioclinic. Dr Moussa has lectured nationally and internationally on topics that include; Hard and soft tissue augmentation around implants, perio-plastic surgery, immediate implants, osseodensification and crown lengthening.

⭕️ A SOFT TISSUE solution for a BONE PROBLEM ⭕️
Here’s an interesting case where I utilized the BD Cuff implant from to manage this horizontally deficient ridge.
This implant has a threaded portion and then a machined collar. I find it useful in sloping buccal defects. Threaded portion is placed in thicker apical ridge portion, while machined collar is level with the thin crestal bone.
I find there to be SYNERGY between SOFT TISSUE and this implant design. Here’s a case example.
1️⃣ CBCT shows. Sloping buccal defect in the posterior mandible. Also missing tissue quantity and quality.
2️⃣ Implant placed with machined collar level with thin buccal bone.
3️⃣ I used a thick CTG as an inlay graft to compensate for volume with primary closure and a HA.
4️⃣ Healing of soft tissues now shows deficient vestibular depth.
5️⃣ I used an FGG + vetibuloplasty procedure to restore the KG and tissue quality.
This case was completed over a year ago and has been going strong. This is not ad but simply some out of the box thinking to better serve our patients.

Implants with a complete lack of attached tissue and zero vestibular depth.
Expensive Gingival Graft (FGG 😂) to the rescue.
1️⃣ Vestibuloplasty first to eliminate the muscle pull and deep vestibule
2️⃣ Partial thickness bed prepared to receive the graft
3️⃣ FGG sutured in position over the vascular bed

⭕️ A SPECIAL CASE ⭕️
This is a very special case for me..even though I did not want to do it in the beginning.
This patient was treated by a colleague of mine with a lower hybrid. Following that, the patient got in to an accident which essentially took out his entire anterior mandible with the implants that are in it.
Patient then got referred to me in hopes of being able to salvage the implant, even though it presents with severe bone loss and recession.
Upon first look at the implant, I did not want to touch it.
But this implant is key to maintain, if there is hope of getting the patient back in a hybrid.
Hers what we did:
1️⃣ Implantoplasty to smooth exposed threads
2️⃣ Tuberosity onlay CTG
3️⃣ Lateral sliding flap to cover
4️⃣ Lingual flap release was KEY
Photos show 4 weeks post-op.
Not perfect, but both the patient and my self were happy with the result.

⭕️ LINGUAL BONE AUGMENTATION ⭕️
In my opinion, guided home regeneration (GBR) is challenging to perform on the lingual aspect. This can be due to several challenges including:
🛑 Difficult access
🛑 Neurovascular structures
🛑 Difficult graft containment
Today I’m posting two cases that show vertical and horizontal GBR on the LINGUAL aspect in the anterior mandible.
Here are some keys that have improved my success in lingual GBR.
🛑 Limited flap reflection and muscle detachment on the lingual aspect
🛑 Using a double layer collagen membrane that completely contains the lingual graft
🛑 Buccal collagen membrane then extends and overlaps the lingual membrane
🛑 And as always, stabilization of the entire graft/membrane complex
With that being said, this is a challenging technique and should be attempted by only clinicians who are thoroughly familiar with LINGUAL FLAP MANAGEMENT.

3D defect in the posterior mandible.
Treated with a KHOURY PLATE horizontal augmentation 4 months ago.
Today was the re-entry with solid bone quantity and quality.
Placed two 8.5mm implants to restore the site.
Next, up soft tissue management.

⭕️ Mandibular FULL ARCH SOFT TISSUE defects ⭕️
One of the challenges of mandibular full arch therapy is KERATINIZED TISSUE DEFICIENCY. Especially when complicated with the presence of failing implants in redo cases.

DECISION MAKING becomes critical in these cases.

How can we accomplish the treatment goals while minimizing the number of procedures the patient has to endure.

Here’s what we did here:
1️⃣ Utilizing a full-partial thickness flap to expose the failed implants and provide a bed for an FGG
2️⃣ Old implants removed
3️⃣ FGG utilized in the anterior mandible to increase the width of KG
4️⃣ Few weeks later we utilize apically positioned flaps to expose the bone and reposition KG
5️⃣ Implants and MUAs placed
6️⃣ KG flaps tailored to fit around MUAs and sutured in place
7️⃣ Photos show healing after temp removal at 10 weeks
Proper DECISION MAKING & SEQUENCING allowed us to repair this case efficiently in two surgeries.

⭕️ SOFT TISSUE management for ZYGOS ⭕️
This patient presented with a SOFT TISSUE DEFECT around a ZYGOMATIC IMPLANT previously placed by my self. Even though I had used buccal fat, patient still developed recession, loss of attached tissue, and vestibular depth.
Here’s what we did to manage this complication.
1️⃣ Lateral sliding flap preparation
2️⃣ Bed preparation for lateral sliding flap
3️⃣ Rotated palatal pedicle used to cover exposed implant surface
4️⃣ Lateral sliding flap sutured over the palatal pedicle
5️⃣ photos show healing at 4 weeks post-op
In my humble opinion…once you understand SOFT TISSUE and its basic principles. The concepts can be adapted to different clinical scenarios including zygomatic implants.

Our Story

Dr Moussa obtained his Bachelors degree in dentistry from The Faculty of Dentistry, Alexandria University, Egypt in 2009. Subsequently, he joined his father in a specialty practice with a primary focus on dental implant surgery.

In 2012 Dr Moussa joined the periodontics fellowship program at Case Western Reserve University (CWRU) in Cleveland, Ohio. Afterwards, he pursued a Masters degree in periodontics and joined the residency program at CWRU.

Recognizing the importance of sound prosthetic principles in a surgical practice, Dr Moussa commenced the fellowship program at Louisiana State University in New Orleans, Louisiana. Throughout the fellowship Dr Moussa focused his efforts on gaining experience in dental implant prosthodontics, and managing complex restorative cases.

Currently Dr Moussa maintains a full time private practice position in New Orleans, Louisiana. His practice is limited to periodontics and dental implant surgery.
