Back from a bit of an IG hiatus 😊
Here’s a recent case we did showing staged implant replacement for a maxillary incisor that presents with a complete loss of the buccal plate. In addition to a narrow alveolar envelope. Photos show steps and details of management.
Wishing everyone a wonderful week ahead ✌️

⭕️ TISSUE TOUCH UP ⭕️
Here’s a follow up on a small tissue augmentation around a natural tooth.
Thin tissue, lack of KG, aberrant frenulum.
Case managed with a tunnel and tuberosity CTG.
Photos show healing at 2 weeks post-op with a completely different biotype. Minimal to no pain reported by the patient.
As I always say…TREATMENT TIMING & SEQUENCE will always affect the TREATMENT COMPLEXITY.

⭕️ BREAD & BUTTER IMPLANT DENTISTRY ⭕️
It’s not always large flaps and fancy grafts…sometimes we keep it simple.
This patient presented with an emergency fracture of #8 along with a relatively thick bio type. Here’s what we did:
1️⃣ Full extraction due to buccal bone loss
2️⃣ Immediate implant placement
3️⃣ Labial GBR with FDBA and a collagen membrane in a flapless approach
4️⃣ Immediate loaded provisional with proper contour management
Photos show final soft tissues along with the final result 👆

⭕️ PATIENT focused TREATMENT ⭕️
Happy Thursday my friends,
This patient presents to us several months back with a desire to restore his mandibular dentition back. Mandibular posterior teeth are missing with severe vertical and horizontal ridge resorption.
On one hand we can remove everything, flatten the bone, and restore with a lower FP3 FULL ARCH.
On the other hand, this patient is young, wants to keep his natural lower anteriors, and very motivated to undergo regenerative surgery.
Putting all these factors in to consideration..and more importantly, treating the patient and not the case..we decided to proceed with posterior bone augmentation and tooth replacement using implant bridges…Not everything has to end up as a hybrid. That way in 10-20 years, if and when this work fails, this patient will still have the hybrid option.
Following the principles is a GRAFT-LESS APPROACH, here’s what we did.
1️⃣ Horizontal ridge augmentation using autogenous bone plates. Same Site harvests. Goal is to build up the horizontal dimension.
2️⃣ Implant placement at 5 months post-op for both sites using short and wide implants posterior.
3️⃣ Addressing complete lack of KG and vestibular depth using Free gingival grafts (FGG).
4️⃣ Second stage surgery with positioning KG flaps 360 degrees around implants.
Will continue to update the case as it progresses!

⭕️ IMMEDIACY PROBLEMS ⭕️
Happy Tuesday my friends,
First premolar immediate implant was performed by my self on this patient. No CTG done at placement.
Here we see the PROBLEMS associated with CONVENTIONAL IMMEDIATE IMPLANTS. Bone and soft tissue resorption resulting in tissue defects like this one.
Unfortunately this defect was both a qualitative and quantitative defect. Utilized a very superficial partial thickness dissection. This was followed by a properly positioned and stabilized FGG.

⭕️Some SOFT TISSUE GRAFTING around implants while we’re SNOWED IN⭕️
Patient was treated with a lower AOX procedure initially. Noted thin biotype and minimal KG at implant placement, and anticipated the need for tissue management at a later stage.
Utilized a simple Free gingival graft (FGG) to increase the quality and quantity of soft tissue after initial healing. Photos show results at 6-8 weeks post-op.
As I say, soft tissue management is not a luxury, rather a necessity in all implant cases…especially full arch.

Happy Sunday 😊
Utilizing SOFT TISSUE GRAFTING to correct anterior recession defects with simultaneous implant placement and provisionalization.
IMHO these procedures require knowledge and skill regarding placement technique, soft tissue management, and proper provisional contours.
