Cannabis Care Clinics of Miami

To qualify for a Compassionate Use Registry identification card, a patient must:
1. Be entered into the Compassionate Use Registry by a qualified physician
3. Be a patient of an ordering physician for at least 3 months
4. Be diagnosed with a qualifying medical condition,
5. Submit a completed application to the Office of Compassionate Use


Qualifying conditions include:

1. Acquired immune defici

ency syndrome (AIDS)
2. Amyotrophic Lateral sclerosis (ALS)
3. Crohn’s disease
5. Glaucoma
7. Parkinson syndrome
8. Post-traumatic stress disorder (PTSD)
9. Multiple sclerosis (MS)
10. Intractable seizures

As well as any other debilitating medical condition of the same kind or class as or comparable to those enumerated, and for which physician believes that the medical use of cannabis would likely outweigh the potential health risks to the patient.
