CWS Vs Exudates Vs Drusen

Cotton Wool Spot:
- White to gray
- Superficial
- Feathery, ill-defined borders
- OCT: localized swelling of the RNFL

Exudates:
- Yellow’ish
- Dot like, fuses together when multiple near by
- OCT: in the middle of the retina, hyper reflective, causes underlying shadowing

Drusen:
- Yellow’ish
- Well defined, can fuse together
- Has a pattern, typically by the macula
- OCT: Below the RPE, uniformly medium reflectivity

Short case-based pearls with the students today.

• CRVO
• Always assess for macular edema with OCT
• Iris exam, watch closely for NVI not just the fundus → neovascular glaucoma

• Rhegmatogenous RD
• RRD often associated with PVD
• Relative scotoma favors RRD
• Absolute scotoma points toward retinoschisis

• Vitreous Hemorrhage
• Commonly secondary to diabetic retinopathy
• Poor view? Examine the fellow eye if suspecting diabetic retinopathy

In a myopic eye, central vision may be well corrected, but relative peripheral hyperopic defocus continues to stimulate axial elongation. The retina interprets this as a cue to grow, and myopia progresses.

Clinical pearls:
• Correcting central refractive error alone does not address the peripheral growth signal.
• Strategies that create peripheral myopic defocus can slow axial elongation during the most active growth years.

Understanding where light focuses matters just as much as how clearly patients see.

Retinoschisis vs Retinal Detachment

Retinoschisis
• Splitting of the retina’s inner layers
• Often congenital or degenerative
• Usually asymptomatic or causes mild peripheral vision loss
• Smooth, dome-shaped, immobile appearance
• ОСТ shows intraretinal splitting
• Rarely progresses or threatens central vision

Retinal Detachment
• Separation of neurosensory retina from the RPE
• Often caused by a retinal tear or traction
• Sudden flashes, floaters, or curtain-like vision loss
• Mobile, corrugated appearance on exam
• OCT shows subretinal fluid
• Urgent and vision-threatening if untreated
