A thoughtful revision can change everything. ✨

This 44-year-old patient came to us after two previous augmentation mastopexy surgeries and a recurrent capsular contracture on the left side — a situation that requires advanced reconstructive skill to correct safely and beautifully.

When you get an implant, your body naturally forms a thin capsule around it—that’s normal and happens in almost every patient. But in a small percentage, that capsule becomes thick and tight, pushing the implant upward and making the breast feel firm or distorted. That’s called capsular contracture.

Sometimes it’s caused by things like infection, bleeding, or radiation… but other times it happens for no clear reason. In those cases, simply replacing the implant isn’t enough—there’s a high chance it will return.

That’s why for recurring or difficult contractures, Dr. Salinas uses biologic mesh to support the implant and dramatically reduce recurrence—from about 50% down to 5%. A long-term solution, not just a temporary fix.

This 55 year old patient was living with extensive breast deformities after undergoing 7 failed cosmetic operations performed by 3 other surgeons 10 years ago.

Her procedures were complicated by multiple infections, open wounds and loss of multiple implants. She had given up and accepted her results, until we met last year after I reconstructed her close friend. With some trepidation, we formulated a plan and returned to the OR. I exchanged her implants, repositioned the inframmamary folds, provided internal bra support and covered the implants with biologic mesh to address the encapsulation.

These difficult revisional cases require a broad reconstructive and cosmetic skill-set. I am thrilled to have finally given her the results she wanted.
