The Residence at Kensington Place is a 109 bed nursing and rehabilitation facility located in Middletown, Ohio. The facility offers 24/7 skilled nursing care with Registered Nurses, IV therapy, secured behavioral unit, Bariatric care, dementia care, hospice care, private short-term rehab suites, specialized wound care program, physical therapy, occupational therapy, and speech therapy services. cility has a dedicated medical team that is comprised of specialized physicians and nurse practitioners. The staff is dedicated to making sure that all residents and families participate in creating a individual plan of care as part of the facility's Person Center Care program. The primary goal of the facility's rehabilitation program is to restore the resident to their highest level of functioning, prior to transitioning into a community setting. The therapy team is available 6-7 days weekly to meet the needs of our residents. Therapists' are committed to ensuring each resident is discharged safely by conducting home visits and discharge conferences prior to day of discharge. Our social service department is dedicated to assisting each resident with making a safe and smooth transition back to their home environment. We assist with contacting community resources such as Ohio Home Choice Program, Meals on Wheels, Home Health Services, and any other social service program available to the resident and families at the time of discharge.
