Do you need your Covid Vaccine? New guidelines indicate that your pharmacy will need a prescription. We will need the following information to process this request.

1. Do you have an appointment set for the vaccine? (If not, please schedule one first)
2. Pharmacy and fax number ?
3. What brand you will be receiving from the pharmacy (Pfizer/Moderna)

These requests can be handled more efficiently by utilizing the patient portal. Alternatively you may call the office with the required information.
