The post below comes from a volunteer firefighter who recently stepped away from volunteer service and into retirement.

While the post is focused on volunteer firefighters the same holds true for our volunteer and call EMS providers.

About three years ago, Milan & Dummer EMS almost dissolved. We were down to approximately three active call members and were responding to just 20% of our 911 call volume.

At that point in time, our board of directors and our towns had to make a choice; continue as we were paying just hourly when members went on calls or make a change. Thankfully we made a change and moved to a paid on call staffing model. While that made a big difference (we have covered 99% of all 911 calls since then), our members get a mere $4.58 an hour today to be available. With the cost of almost everything going up exponentially the last couple years, when our members are forced to choose between putting food on their tables by working more hours at their day jobs vs. being on call for us, that's not much of a choice.

Our changes incrementally have been working. In 2023 Milan and Dummer EMS answered approximately 21 calls for service. In 2024 that number was 131+, in 2025 241 calls answered. Not only is our agency covering more calls than ever before, but we are doing it with the largest number of members we have had on our roster in the last ten years. There is more still work for us to do all, and for that we need the communities help.

The lowest level of licensed provider on an ambulance needs a minimum of 140 hours of education with our more advanced providers requiring north of 500-1500 hours of education. This doesn't even factor in the years of applied experience it takes to be proficient. Our members are professionals that are highly trained, well educated and experienced emergency medical providers who show up when minutes matter. We need to come together to support them and the work that they do.

We are always looking for more individuals looking to help. Even if you don't hold a license, but have a desire to help, we will train you. Let's not let community service and volunteerism die in our area.

——————————————————-

Well, today I finally sat at work and found some words to explain my feelings. In my 26 years of of public service fire and 23 for EMS, the service has changed so much in so many different ways. It’s lengthy, but to anyone volunteering in emergency services, this will hit home.

Over the last couple of weeks, I’ve been asking myself if people truly realize how blessed some cities are to have a paid fire department or rescue squad. Volunteer firefighting is disappearing, little by little, every day across the nation. People simply don’t wanna do it anymore.

Communities expect firefighters to give, give, and give — because somewhere along the way it became normal to believe the job shouldn’t be paid.

Picture this: it’s 2:00 AM. You’ve worked your full-time job all day. You have a newborn at home and a spouse who’s exhausted and upset because you’ve barely been there. You’ve been asleep for what feels like five minutes when the tones drop. The radio wakes the whole house. Baby crying. Spouse frustrated. Your eyes burn, but you get up anyway.

You throw your clothes on and run out the door like it’s your own house on fire — because seconds matter. You drive your personal vehicle, on your own gas, eight minutes down the road to the station. You pull on your turnout gear, climb into the truck, and head toward the scene.

On the way, something sinks in your stomach — you’re the only one responding.

You ask dispatch to page it out again, just hoping for one more set of hands.

Silence.

Fifteen minutes later, you arrive to heavy smoke and fire showing from the front of the house. A woman stands in the yard — half-dressed, barefoot, frantic — because her husband of 40 years never made it out after going back in for the family pet.

You’re one person.

You call for mutual aid. The nearest department is 20–25 minutes away on a good night, and even then you might only get two more people — people who care deeply, but whose bodies just can’t keep up anymore.

Now you’re alone — running the pump, checking for hazards, stretching lines, and trying everything in you to fight a house fire by yourself. You’re carrying 85 pounds of gear, an SCBA, tools, and a charged hose line across the yard. You try to push inside, but the heat is overwhelming. You need another line. You need backup.

You don’t have either.

The truth is, the closest help might still be 25 minutes away — if it’s coming.

You want to go inside more than anything — to reach the husband, the father, the papaw, a man who is everything to his family — but you can’t.

You’re there for hours — fatigued, mentally broken — beating yourself up for not being able to get there quicker. You ask yourself if you could’ve done something different to change the outcome. But there’s no time for that. It’s 7 AM, and you have to be at your full-time job at 8.

This is the reality people don’t see when they say firefighters shouldn’t be paid — when they forget that behind the sirens are human beings with families, fatigue, fear, and hearts that keep showing up anyway.

Volunteer firefighting is a calling that is quickly diminishing not only across New Hampshire, but across the United States as a whole. The men and women in your communities give their whole lives to serve and expect nothing in return.

Support your local fire departments. If your community is fortunate enough to have a paid and fully staffed department, you are blessed with firefighters and rescue personnel who can respond immediately — rested, trained, and ready around the clock. Paid staffing means faster response times, more firefighters arriving together, quicker and more efficient rescues, and better protection for both citizens and firefighters. Advocate for funding, training, and adequate staffing so departments can put enough boots on the ground when seconds matter. Thank them while they’re still here to hear it, and teach your children that service matters — and that the people who run toward danger deserve real support, not just words.

Because without community support, there will come a night when no one is left to answer them tones.

Happy Holidays from all of us at the Milan & Dummer Ambulance.

Thank you to our mutual aid partners Gorham NH Fire & EMS, Errol Firefighters Association, Errol Rescue, 45th Parallel Emergency Medical Services, Professional Firefighters of Berlin IAFF Local 1088, Emergency Management - Town of Milan, NH, Berlin NH Police Department.

A special thank you to our board of directors who share our vision to provide cutting edge and lifesaving care to our communities for all of your support this year.

Thanks to the New Hampshire Charitable Foundation, Harbor Freight, Tillotson Foundation, and Wyman Family Fund for the support we received this year in the form of grants that funded lifesaving advanced life support equipment for our second ambulance.

Finally thank you to all the members of both the communities we serve as well as the neighboring communities for all your support, it means the world to us and our members.

We appear to be on track to finish the year with around 250 calls for service. This exceeds our previous highest call numbers of 131 calls last year, making it almost a 100% increase in volume.

As we head into 2026 we are excited for our 5 members who have taken or are taking the Advanced EMT Exam, 7 Emergency Medical Responders who are also getting ready to test, and one EMT student all of whom have devoted a combined 3000+ hours of education and clinical time obtaining their licenses.

If you want to join us in serving the community, we are currently planning a late spring EMR class to be hosted locally. If you have any interest in joining the class, please email admin@mdamb.com for more information.

Today wrapped up the accelerated Emergency Medical Responder class hosted in Stark.

Students completed over 140 hours of educational requirements and clinical skills.

Our cohort now moves on to taking the national licensing exam. We can't wait for them to start their orientation ride time on the ambulance as newly licensed providers.

Congrats to all of our students, and a big thank you to Deb Boyle from Becker Training and our agencies co-instructors who made this class possible.

We hope to offer another EMR class in the spring. If you have an interest in serving the community, please reach out to us to secure your spot in our next class.

Today we recognize the 21+ responders of Milan & Dummer EMS along with our 6 new members currently training to serve their community.

Our members spend countless hours away from their loved ones to answer the call for those in need.

This year has been one of transformation for our members and our communities. Our call volume has increased 4X over where it was just a few years ago.

With that, we added a second ambulance back in April to help cover the additional volume. Now we need the community's help. With such drastic increases in call volume, we are looking for individuals within the community who are interested in becoming emergency medical responders. We plan to host a class fully paid for by the agency in the Spring for those who would like to get involved and make a difference.
