PACE Your LIFE

PACE Your LIFE is the first Program of All-Inclusive Care that is currently serving southern Kent County and northern Sussex County.

The Program of All-Inclusive Care for the Elderly (PACE) gives participants the ability to stay in their home while receiving essential services and maintaining independence and physical and emotional well-being. PACE Your LIFE provides comprehensive medical and social services to adults 55 and older that reside in the community, requiring assistance and mostly to individuals who are eligible for Medicare and/or Medicaid benefits. A team of healthcare professionals provide PACE participants with personalized medical care.

It's the perfect time for an afternoon of FREE Self Care! You'll also have the opportunity to meet our staff, tour our Day Center, and learn more about how we are helping local seniors to live safely and independently at home!

These are the everyday ways our PACE community shows what caregiving really means. It’s not always formal. Sometimes it’s helping a friend build a birdhouse. Sometimes it’s making someone laugh on a tough day. Sometimes it’s just being present.

At PACE Your LIFE, caregiving is woven into everything we do. Participants, staff, families, and friends all have a part in creating this circle of support.
