Transition of care is optimized by a comprehensive team, which, in addition to the patient, may include family, physicians, nurses, therapists, social workers, educators, legal representations, and advocates. This course will explore these roles further, illustrate some of the pitfalls of mental health transition in this patient population, and outline strategies for optimizing transition of care.
