Key Findings:

The idea that pelvic incidence (PI) may not always be a fixed parameter is truly thought-provoking—especially in light of recent evidence.

A new article reviewing 13 studies involving 1,055 adult spinal deformity (ASD) patients found that while the average PI remained stable, over 45% showed a change >5° postoperatively.

Key contributing factors included:

1. Fusion to the sacrum
2. Iliac screw use
3. High preoperative sagittal vertical axis (SVA)
4. Greater PI–LL mismatch
5. Lower preoperative lumbar lordosis
6. Increased pelvic retroversion
7. SI joint laxity/instability
8. Extreme PI values
• Low PI tended to increase post-op
• High PI tended to decrease post-op

These findings open the door for more tailored sagittal realignment strategies and deeper exploration into PI dynamics in ASD patients.

Check out this brief article review on the article, "Zones where reduced implant density leads to correction loss after scoliosis surgery for Lenke 1A adolescent idiopathic scoliosis: a multicenter study" from the Spine Deformity journal!

Key Findings:
▫️Increasing rod rigidity tended to reduce the likelihood of correction loss in both convex and concave curves

▫️Implant density reduction in the convex apical and peri-apical zones were independent predictors of significant correction loss.

Conclusion: Low implant density in the convex apical and peri-apical zones were significant factors predicting correction loss at 2 years postoperatively. However, the difference in correction loss between groups may not have been large enough to be clinically meaningful.
