It's Breakthrough Friday, your weekly look at the science transforming futures for patients who can't wait.

This week: a finding that could change how sickle cell gene therapy is designed.

Scientists knew that a protein called fetal hemoglobin can protect sickle cell warriors. What they didn't know is that it matters how evenly that protein is distributed across a patient's cells, not just how much of it is there on average.

VBRI's Dr. Phillip Doerfler and colleagues found that patients with consistent fetal hemoglobin across all their cells had no symptoms, whereas those with uneven levels experienced pain events, even when their average levels were similar.

James was diagnosed with sickle cell disease at age two. He spent years of his childhood in and out of hospitals, enduring pain he describes as "a jackhammer going through my body," infections, surgeries, and milestones marked from a hospital bed.

He's 44 now. He works as a medical assistant at Children's Hospital, showing up every day for patients who need the kind of care he once depended on.

James is one of approximately 100,000 sickle cell warriors in the United States. He is the reason research can't wait.

Most multiple myeloma patients are told they'll manage this disease for the rest of their lives. A recent study by VBRI researchers aims to provide a different answer.

By studying patients who achieved long-term survival, the study, authored by Khanmi Kasomva, PhD, and co-led by VBRI Senior Investigator and Interim Co-Director Sid Rao, MD, PhD, found that their immune systems share distinct strengths: better T-cell activity, stronger natural killer cells, and fewer cells that suppress the immune response. That immune environment appears to hold the cancer in check.

If we can create that environment intentionally, more patients could achieve sustained remission, rather than just longer management. Families deserve answers today. That's why relentless research can't wait.
