Learning Objectives:
1. Discuss clinical trial evidence of targeting inflammation in human atherosclerotic cardiovascular disease.
2. Discuss clinical trial evidence of targeting metabolic inflammation in human heart failure.
3. Understand fundamental ways in which the immune system and cardiometabolic risk factors interact to result in excess, harmful inflammation versus resolution thereof.

A groundbreaking new study published in the journal Molecular Therapy heralds a major step forward in treating muscular dystrophies. The research, led by LHI researcher Rita Perlingeiro, PhD, details a clinically-ready process to generate therapeutic cells for muscle regeneration.

Dr. Perlingeiro’s team successfully developed a current good manufacturing practice (cGMP) compliant protocol to create large-scale, cryopreserved PAX7-induced myogenic progenitors (MyoPAXon) from induced pluripotent stem cells (iPSCs). This is a critical development, as it addresses the scalability and safety requirements needed for human clinical trials.

The study, titled "Preclinical quality, safety, and efficacy of a cGMP iPSC-derived myogenic progenitor product for the treatment of muscular dystrophies," demonstrates that MyoPAXon can effectively reconstitute both new muscle fibers and muscle stem cells (satellite cells) in preclinical models. This robust regenerative capacity positions Dr. Perlingeiro's work as a pioneering effort toward the first-ever stem cell-based treatment for devastating muscle-wasting diseases.

Exciting news for the pulmonary hypertension (PAH) community! Our new publication, "Microbiota Transplant Therapy Is Safe and Feasible in Pulmonary Arterial Hypertension," is now live in JACC: Basic to Translational Science. This marks a significant step forward as it's the first human pilot trial to study microbiota transplant therapy (MTT) in PAH patients.

The study enrolled 11 patients and found that MTT is safe and feasible, with no serious or severe adverse events related to the treatment. We observed that the therapy was associated with a transient reduction in circulating proinflammatory cytokines and a modest level of donor bacterial engraftment. While the study did not show statistically significant changes, we did observe positive trends, including an increase in the six-minute walk distance and an improvement in quality of life scores.

These findings suggest that modulating the gut microbiome may be a promising new therapeutic strategy for PAH. We are hopeful that this research will pave the way for further investigation into new treatment options for patients.

Please join us in welcoming Dr. Selma Carlson to the Division. We’re thrilled to have her on board and look forward to the expertise, energy, and dedication she will bring to our team.

Dr. Carlson completed internal medicine residency and cardiology fellowship at University of Washington in Seattle, and advanced multimodality cardiovascular imaging fellowship at Minneapolis Heart Institute. Her clinical interests include hypertrophic cardiomyopathy, aortic disease, preventive cardiology, and women’s cardiovascular health. Her research efforts are focused on the clinical application of cardiac CT and stress CMR for patients with coronary artery disease, use of cardiac CT in planning of complex structural interventions, and the role of multimodality imaging in women’s cardiovascular health. She also has an interest in cardiac imaging fellowship training/education.
