What if your period pain isn’t coming from your uterus at all?

Period pain isn’t something you have to live with. Believe it or not, you can influence it (I, Dr. Julie, am living proof).

Two commonly overlooked drivers of cramps actually start outside your uterus:
✔️ your abdominal fascia
✔️ your body’s global inflammation level

👉Fascia
Tight abdominal fascia can amplify cramps and nerve pain.

Why? Because fascia communicates with your organs and nerves. When it becomes unhealthy, pain can get worse as it will inflame any structure that sits below it or it surrounds (think pelvic nerves and the uterus itself).

Myofascial release (like our skin-rolling vids!) can change this process. I used these exact techniques myself to reduce severe period pain.

👉Inflammation
Chronic inflammation often comes from an imbalanced Omega-6 to Omega-3 ratio. Ideally, you want a 1:1 ration, however, the average American diet is closer to 20:1 😳.

This major imbalance drives more pain.

Fish oil is a simple solution to improve this ratio. In fact studies show high-dose fish oil can work just as well as 800 mg of ibuprofen in women with period pain. I take 4,000 mg fish oil every morning… total game changer.

💫 Let’s talk Connective Tissue (CT) restriction - AKA your superficial fascia!

This is characterized by dense subcutaneous thickening that can be uncomfortable during a skin rolling examination.

👉 In a PF PT assessment for those with pelvic pain & s*xual pain, we always evaluate the CT, following fascial trains from foot to pelvis to head (cranium) 🔍

When CT/Fascial lines are restricted, there’s:
👉 decreased blood flow through the area
👉 increased inflammation on any organ, nerve, or muscle below it
👉 increased tissue sensitivity to touch.

😌 These changes set the stage for difficulty with everyday functioning such as wearing tighter clothing (pants & underwear especially), sitting, bladder filling, and having pain-free s*x.

Pelvic pain syndromes almost always have this component present! 📋

Your glutes aren’t just for aesthetics… they’re essential teammates in pelvic floor rehab.

🍑 Glute Maximus
The engine of hip extension. Weakness here means other muscles (like your pelvic floor!) start picking up the slack.

🍑 Glute Medius
The single-leg stabilizer. It keeps your pelvis level with every step you take. When it’s underactive, we often see asymmetries that show up as hip, back, or pelvic pain.

🍑 Glute Minimus
The subtle stabilizer. Deep and often forgotten, it helps rotate and steady your hip joint, especially important in people with pelvic pain or SI joint dysfunction.

In pelvic floor PT, treating the pelvis means treating the muscles around it too. Don’t skip the glutes! They’re part of the team.💪

Your pelvic floor is strong. It’s just tired of doing your glutes’ job too!

When your glutes are underactive (or MIA altogether), your body doesn’t just stop moving, but it finds other ways to get the job done. The inner thighs take over. The low back pitches in. And your pelvic floor has no choice but to start working overtime to keep things stable.

Over time, this creates imbalances that lead to symptoms like leaking, heaviness, hip pain, low back pain, and more.

The fix? Stop asking your pelvic floor to do it all. Start retraining your glutes to show up and support you the way they’re meant to.

Did you know that your pelvic floor muscles are directly attached your hip muscles?

That’s right, the hip bone is connected to the pelvic bone and the pelvic floor muscles are connected to one of your hip muscles: the Obturator Internus (OI)!

If the OI becomes strained, tight or weak you may experience:

❌Pain in the outside of the hip
❌Pain with sitting
❌Deep pelvic pain
❌Lower abdominal pain
❌Groin pain
❌Urinary urgency/frequency
❌Urinary leakage
❌Sexual dysfunction

Restoring mobility and function of the OI, pelvic floor, and other hip muscles can help to resolve your pain and get you back to doing all the things you enjoy.

Front of the hip hurting? It might be your nerves.

Not all anterior hip pain is from muscles or joints. Sometimes, it’s nerve-related and often overlooked. Here are a few key players:⁣

🧠 Genitofemoral nerve – sharp or aching pain in the groin, inner thigh, or testicle/labia.⁣
🧠 Ilioinguinal nerve – pain or tingling along the groin crease, often after surgery or injury.⁣
🧠 Iliohypogastric nerve – discomfort near the lower belly or side of the hip.⁣
🧠 Femoral nerve – pain or weakness in the front of the thigh.⁣
🧠 Perineal nerve – sitting pain that wraps to the front of the pelvis.⁣

If your hip pain isn’t improving with stretching or strengthening, it might be time to look deeper. Nerves matter!

What does pelvic pain look like in men?

It’s more common than you think… and often overlooked.

Men with pelvic pain are frequently dismissed, misdiagnosed, or too uncomfortable to speak up. But the symptoms are real and deserve attention.

A few symptoms we see often in the clinic are:

🔹 Groin pain (that isn’t a hernia)
🔹 Testicular pain that comes and goes or feels deep and hard to localize
🔹 Low back pain that doesn’t resolve with standard care
🔹 Sitting pain that worsens over time or radiates into the perineum

These symptoms can signal pelvic floor dysfunction, nerve irritation, or chronic pelvic pain syndrome (CPPS). Too often, men are told “it’s just in your head” or “it’s normal for your age.”

Continuing on the theme of posterior hip pain, lets dive into the nerves!

Posterior hip pain isn’t always coming from the muscles or joints, it could be the nerves too!

A thorough assessment should consider:

🔹 Cluneal nerves: can mimic SI joint or glute pain
🔹 Pudendal nerve: think burning, tingling, or pressure in the perineum
🔹 Posterior femoral cutaneous nerve: causes posterior thigh burning/numbness
🔹 Sciatic nerve: not just sciatica – track its full path
🔹 Pelvic splanchnic nerves: linked to pelvic organ pain/dysfunction
🔹 Obturator nerve: medial thigh pain, especially during movement
🔹 Superior/inferior gluteal nerves: motor nerves that affect glute strength and pain
🔹 Lateral femoral cutaneous nerve: may refer pain laterally but radiate posteriorly

Nerve pain can be elusive. Look past the obvious to get to the root.

Trying to figure out what’s behind your stubborn glute or posterior hip pain?

It’s not always straightforward… and often, it’s not just about the hip.

This area is a busy crossroads for multiple structures that can mimic each other when irritated or overloaded. Some common culprits include:

– Lumbar spine referral
– Hip joint pathology
– Obturator internus (a deep pelvic floor muscle)
– Adductor tendinopathy
– Ischiofemoral impingement
– Pudendal neuralgia
– Sacral stress fracture
– SI joint pain
– Gluteal tendinopathy
– Piriformis syndrome

Many of these diagnoses can overlap with pelvic floor dysfunction. Why? Because the hip, pelvis, and pelvic floor are structurally and functionally linked.

For example, a tight obturator internus or overloaded adductor may affect pelvic floor muscle tension or coordination—contributing to symptoms like tailbone pain, pelvic pressure, or even pain with s*x or sitting.

Pain in this region varies person to person. It might show up as sit bone pain, deep buttock ache, pinching with hip movement, or nerve-like symptoms. And while it’s tempting to chase the exact pain spot, location alone rarely tells the full story.

A thorough pelvic PT evaluation considers all of this, helping you uncover the true driver, rather than just treating symptoms.

If you’ve been stuck in a cycle of glute pain and pelvic discomfort, don’t give up. There is a way through. It starts with zooming out and looking at the whole picture.
