Hy-lond Health Care Modesto is seeking an experienced MDS Nurse for a Resident Assessment Coordinator position!

Qualifications:

Must have a current, active license to practice as an Registered Nurse (RN) in State of practice.
Must be well-versed in inter-personal communications.
Must be familiar with the RAI process and Case Management.
Must be able to read, write, and speak English.
Must meet all local health regulation and pass post-employment physical exam if required. This requirement also includes criminal background investigation and reference inquiry.
