We are seeking Caregivers/Home Health Aides to become part of our dynamic team! You will provide ADL assistance to consumers in the consumer's home setting.

Responsibilities:

*Provide routine individualized care to the elderly, convalescents, or persons with disabilities
*Monitor and report changes in consumer health status
*Provide personal care, such as bathing and dressing
*Provide individualized care following a modified care plan
*Medication maintenance

Qualifications:

*Experience in hospice, home health, or other related fields
*Positive and patient demeanor
*Ability to build rapport with consumers
*Ability to handle physical workload
*Excellent written and verbal communication skills
