New substance showing up in the Pittsburgh-area drug supply: Medetomidine

Medetomidine is a highly potent veterinary sedative (stronger than xylazine/“tranq”) that’s increasingly being found mixed into fentanyl and other drugs supplies.

Why this matters: Overdose can look different.
✅ Medetomidine is not an opioid, so someone may not “wake up” quickly even after naloxone. Keep monitoring and support breathing.
✅ Naloxone (Narcan) still matters because fentanyl or other opioids are often present — give naloxone right away.

If someone might be overdosing, here is what to do:
✅ Call 911
✅ Give naloxone and repeat doses as needed
✅ Rescue breathing (slow breaths if they’re not breathing normally)
✅ Stay with them — sedation may last longer; keep them on their side if breathing returns

⚠️ Withdrawal can be more severe and complicated. Withdrawal symptoms in people using medetomidine present substances included severe autonomic symptoms (like very high blood pressure, fast heart rate, heavy sweating/tremor, vomiting) and often required ICU-level care.

If withdrawal symptoms feels “different” or extreme (severe agitation, nonstop vomiting, chest pain, dangerously high BP/HR, confusion), go to your nearest ER right away.
