Attention.📢 We are ringing in the New Year with a change of schedule! While Admin Day is currently on Tuesday, come first of the year, it will be on Monday.

2024 hours will be Tues-Friday 8:30am - 4:30pm.🎉

Welcome to Chronic Disease Management, LLC. 👋

This medical practice was founded on the belief that patients deserve more than what "corporate medicine" is currently providing them. I spend the time it takes to listen and understand where you are, then we go forward from there.👍

I accept all patients over 18 years of age but specialize in helping you manage your chronic condition, which CDC defines as:
"Chronic diseases are defined broadly as conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both." This would include simple problems like high blood pressure, diabetes, thyroid, heart or lung disease (a-fib, asthma, etc).

We are located at 7 Microwave Hill Rd, Ste C (literally right next door to Montana City Fitness).📍

We are contracted with most insurance companies, and we have a serious discount plan if you do not have insurance. Call and leave me a message if you have other questions.☎️
