To our patients needing to transfer records : please complete the records release form on our website and email it back to us. (Info@bullockmedspa.com) due to the influx of records releases, we ask that you please allow two weeks for the records to be sent. This form needs to be filled out in its entirety ( including the receiving doctors’s phone and fax number) if the form is not filled out completely, this will result in a delay in receiving your records.
