Welcoming Dr. John A. Fleishman, M.D.

We are very pleased to have John A. Fleishman, M.D. join our practice. Dr. Fleishman is a graduate of Case Western Reserve University School of Medicine, and he completed his ophthalmology residency and a fellowship in neuro-ophthalmology at the University of Michigan. Dr. Fleishman is a long-standing member of the American Academy of Ophthalmology.
Dr. Fleishman and his wife Leslie, a Principal Harpist with the Dayton Philharmonic, enjoy the arts and spending time on their farm and with their granddaughter, Zoey.
Dr. Fleishman has over 30 years of experience in the practice of ophthalmology and neuro-ophthalmology, and is available to see and treat patients for medical and neuro-ophthalmic diseases of the eyes.

A 64-year-old woman states that she has trouble reading with her glasses while lying in bed.

Is the problem cataracts? Does she need stronger bifocals?

Many people like to read while lying in bed. In order to see through the bifocal portion of the lenses without having to tilt your chin upward, you have to be in a position in which you can look downward through the bottom of the lenses. Since most people are lying almost flat while reading in bed, they are generally looking through the middle or top portion of their glasses, not the bifocal portion at the bottom, making it difficult to see smaller print. A solution to this is to use reading glasses, either prescription or over the counter, in which the entire lens is set for reading. This allows the person to lie in any position and still be able to read (that is, until they fall asleep!).

COVID-19 Vaccine and Dermal Fillers

There have been reports of facial swelling in areas of previous cosmetic dermal filler placement in a small number of patients receiving the COVID-19 vaccine. The reaction has been limited and no serious adverse effects have resulted. Swelling in areas of previous filler placement has been noted to occur with other events including dental or sinus infections, as well as the influenza vaccine.

The reactions are very temporary and respond to treatment with corticosteroids. Thus, a history of receiving eyelid or facial fillers should NOT preclude someone from receiving the COVID-19 vaccine, and it appears safe to receive fillers even after receiving the vaccine.

A woman undergoes cataract surgery and within a few days, notes pain in her left foot. Could the cataract surgery have caused left foot pain? What about her eye drops?

Achilles tendinitis and rupture is a known complication of the fluoroquinolone class of antibiotics. These include ciprofloxacin (Cipro), levofloxacin (Levaquin), moxifloxacin (Avelox), gemifloxacin (Factive), and ofloxacin (Floxin). These antibiotics are given as pills and through IVs for certain bacterial infections. They are very effective and commonly used as eye and ear drops as well. It is extremely rare to develop any tendon issues as a result of fluoroquinolone drop use because the concentration absorbed into the rest of the body is so low, but it has been reported to have occurred. If someone has a history of tendon issues with previous use of a fluoroquinolone antibiotic pill or IV, they should avoid using any fluoroquinolone drops if possible.
