Subject is 44-year-old male, married, 2 children 7 & 9, disabled with chronic pain since MVA 9-16-09. Presenting 6-2- 2014 for initial Neuroenergetic Release (NER) evaluation and treatment.
Primary complaint: Neck pain constant since MVA 9-16-09. Migraine headaches, debilitating 1x/2wks .
L SI pain radiating down L glute, lat. Thigh, lat. leg. B LU/SA tenderness and pain. B ant. hip/groin pain, Difficult to stand or walk for length of time > 10 min.
Secondary complaints: Occasional dizziness, chronic decreased balance, indigestion, intestinal pain, constipation,
History: MVA 9-16-09. Subject reports ruptured discs C5-6, L4-5, L5-6.
Medication: Oxycodone, flexeril, fioricet, Xanax. Subject reports trying many modalities/practitioners (chiropractic, acupuncture, massage, physical therapy, allopathy) to no avail to relieve chronic pain.
Treatment:
6-2- 2014 initial treatment: presents with migraine, P head/neck/B shoulder pain, B Sa/gluteal pain, A hip/groin pain, I intestinal discomfort
Follow-up appointments 6-5-14, 6-9-14, 6-16-14, 6-20-14, 6-27-14,
6-5-14 Reports migraine gone since TX 6-2-14, increase smell/taste, less sinus congestion, no L SI/Sciatic pain, decreased depression and anxiety. Less dizziness, increase balance and strength. Sleep good. Increase mental focus and concentration.
6-9-14 Reports less sinus congestion, sinuses draining, increase smell/taste, no L SI/Sciatic pain, decreased depression and anxiety. No dizziness, increase balance and strength. Slept great. Diarrhea evening od Tx 6-5-14.
6-16-14 Detox soreness all over day of last tx. Sinuses draining. Some B Sa tenderness/soreness and tightness B cv/S th after bending/packing to move.
6-20-14 Reports detox/soreness less last tx. Increased energy/stamina after last visit. Able to lay on back in bed 1st time in 5 years. Holding improved posture. No migraines and has stopped meds since starting NER 6-2-14 . Lots of lifting for moving, some B gluteal soreness, mild to moderate.
6-27-14 Reports B gluteal soreness improved after last Tx. Continued bending/packing/lifting for move. 4 days ago neck sore /migraine relatively mild. Last 3 days I B gluteal pain, L hip thigh weakness wt. bearing, limping. No b glute pain/limp after treatment.
Subject Moved to Florida to live with relatives/ reduce costs. 2014 Several phone coaching sessions. Subject able to continue recovery. Able to exercise, walk bike.
March 2016 Subject traveled to CO to do 4 sessions to continue treatment.
July 2016 Subject moved back to CO to be near to continue treatment. Subject got off disability, returned to work doing title insurance.
Rest of 2016 4 follow-up NER treatments.
2017 2 maintenance/wellness NER treatments. Subject has purchased a home and is fully functional.
