Strange place for a tumor! Who would think? The top of the fibula! A Giant Cell Tumor. Most of these are dismissed as sports injuries for awhile and treated conservatively until an Xray is taken.

How can we tell this is benign?

Even though majority of bone is destroyed the junction of the tumor with normal bone is sharply defined meaning this is a geographic pattern of bone destruction.

The soft tissue mass also has an egg shell rim of calcification around it meaning the tumor is growing slow enough for the periosteum to react and try to contain the tumor.

Lastly GCT of bone is usually associated with internal trabeculations seen here. There are 6 tumors that demonstrate internal translations.

The fibula is a rare site for a giant cell tumor of bone. Most occur in the 20-40 year age group. They arise in metaphysis and spread to epiphysis. They are end of bone tumors.

They are not associated with calcifications.

The lower femur, upper tibia (around the knee) sacrum and distal radius are most common sites. They are benign but aggressive. Perhaps 1 percent can spread to lungs or be found in other bones.

This was approached through a lateral position and incision.

Extensive exposure with a meticulous dissection and separation of peroneal nerve and all branches and the popliteal vessels. It was resected en bloc with the opposing facet on tibia.

The soft tissues and lateral collateral ligament were reconstructed with the biceps femoris and a suture anchore into the upper tibia with knee in 20 degrees flexion.

Postop: No foot drop; Stable normal knee function and no recurrence.
