15 year follow up! I am extremely fortunate to have patients follow with me for so may years. Its incredibly fulfilling to watch them grow up and age gracefully and beat their disease! Forever grateful that they placed all their trust in me to do their complex surgeries and save their limbs and help save their lives! This lovely patient had a huge secondary Chondrosarcoma coming from her scapula. An extremely rare location. We removed the cancer and scapula with a limb saving resection and reconstructed it with a total scapula prostheis. The prosthesis remains in place without loosening providing her with a stable shoulder and good hand and elbow function. It certainly beats a forequarter amputation and in my opinion a flail shoulder if removed and just left hanging. She is doing great. I rember her case so well as she traveled from Ohio and toward the end of her surgery the hospital caught on fire. We had ti remain in the room with her intubated for a couple of hours until the fire was controlled and was safe to leave. I have a vivid memory of her remaining safely intubated and bandaged and us congreg*ting in the corner of room with surgical masks on as smoked filled the room.

Took care of this amazing young man 15 years ago when he was only 3 years old and developed a Ewing Sarcoma. A very unusual age to develop a Ewing sarcoma. Thank God he was a husky baby. The customs department at the major orthopedic manufacturers was shut down so I had to implant an adult modular segmental tumor prosthesis that is still inside him today. To maintain as equal as possible keg lengths he underwent periodic operations to remove the internist piece and place a longer one. Fortunately only 15% growth of the femur comes from the proximal growth plate. I eventually stopped the growth on the opposite side. He’s a happy kid and is always great to see him and his mom. I am grateful and appreciative his mom put all her trust in me to perform this complex surgery on her baby to save his leg and help save his life.

Synovial Sarcoma! Another one of the many faces of
This is an intraarticular synovial sarcoma of the knee. It was read on an MRI as a possible large nodular PVNS (benign condition). While the majority of synovial sarcomas do not occur in an intra articular location it is important to remember it as a possibility as to always consider it in the differential. Synovial sarcomas have a broad radiological appearance. They can appear similar to a cyst or a hematoma. They occur mostly in the deep tissues but can occur in the skin layers and in joints. They often grow somewhat lobular and can be very tiny or very large. They may calcify. They can have a triple signal intensity on MRI with high, low and intermediate signal intensities. Fifty percent are painful. Patients may give a history of having a palpable mass for many months or even a couple of years. They occur in young and older age groups and is likely the most common is adolescents and young adults. They can be graded as low, intermediate or high grade. Histologically they stain positive for vimentin and actin. Surgery is the mainstay of treatment. Radiation and chemotherapy may be indicated depending on size, location, grade, age of patient and other comorbidities.

Awareness! Lumps and bumps in children can be bad! Sarcomas are usually painless and grow in all age groups and all locations. This is a story of a two year old little boy where a sarcoma was missed for a period of time. The child ultimately had an amputation. It’s important to spread the word and make the public aware of these rare cancers. Be the Alchemist! More reflection as we move into 2026!

One of the most horrible days of my life! Changed my life forever! But ended up being a driving force for focusing on my practice and saving the limbs and helping save the lives of so many children and adults with bone and soft tissue cancers. A positive thing that came out of one of the most neg*tive events in history that I had no control over. I surrendered and let God and the universe do their work.

September 1, 2001, marked the start of a new chapter in my life. I had just begun my practice as an orthopedic oncologist at NYU Hospital for Joint Diseases and Bellevue Hospital. Ten days later, that chapter was forever altered. ounds of collapse.
Crushed cars lined the streets. The air was thick with the smell of smoke, dust, and death.
I remember stepping on a torn page from an encyclopedia. A section on Bone Tumors, my field. In that moment, it felt like a sign, a reminder that even in such darkness, I had a responsibility to use my gifts for good.
When I returned home the next morning, I sealed away my scrubs, my shoes caked with dust, and those encyclopedia pages in a bag. For twenty-one years, I could not bring myself to open it. The memories were too raw, the images too vivid. But in 2022, I decided it was time. I donated those items to the 9/11 Memorial & Museum so that the story of that day, and the countless lives it touched, would never be forgotten.
What haunts me most are not only the images of destruction but the aftermath.
Every day, on my way to work, I passed blocks lined with shrines, photos of the missing, flowers, candles, and desperate families waiting for answers. They would stop me, show me pictures, grab my hands, and ask me to pray with them. I could see the trauma etched on their faces.
