We provide family practice care, including geriatrics, adolescent and pediatric care. Our facility is subdivided into 3 separate practices- Koch Family Medicine, Koch Physical Therapy, and Restore Medical. In conjunction with the 3 practices; we have digital x-ray, 2 procedure rooms for minor surgeries and musculoskeletal injections. Koch Family Medicine is a primary care facility with 4 providers
; Stanley Koch, MD, Kimberly Ford, DO, Angela Gilles, APN CNP; and Mary Imig, DO. Our goals are to provide ongoing personalized health maintenance and monitoring of chronic conditions such as blood pressure, diabetes, cholesterol and many more. In addition to seeing patients in office, Dr. Koch and Angela make routine visits to area nursing homes. Dr. Ford directs Safe Haven Hospice. Koch Physical Therapy is a full-service physical therapy and rehabilitation center. Staffed by Jonathan Opperman, DPT, Mary Gerig, PT, and Rachel Koch, PTA. Services include treatment and education following orthopedic surgery, injury, and ongoing care for osteo/rheumatoid arthritis. Restore Medical, run by AJ Cummings, MD and Stanley Koch, MD, specializes in occupational health, injury and rehabilitation, as well as independent medical exams for worker's comp. cases or other legal purposes.
