Wabash Community Health Center is a non-profit organization dedicated to providing quality, affordable health care to all members of our community - which means we treat patients regardless of their ability to pay for services. Our health center provides primary care and behavioral health services and is located in Mt. Monday: 8 AM – 6 PM
Tuesday: 8 AM – 7 PM
Wednesday: 8 AM – 6 PM
Thursday: 8 AM
– 6 PM
Friday: 8 AM – 12 PM
If you are having a crisis emergency after business hours, please go to the nearest emergency room or call 9-1-1. If this is a medical emergency, please go to the nearest emergency room or call 9-1-1. To speak with the nursing supervisor at Wabash General Hospital, please call (618)262-8621. If it is not an emergency and we are busy assisting another patient and cannot answer during normal business hours, please leave a message and we will promptly return your call within the hour. If your message is left after hours or on the weekend, we will promptly return your call the next business day. We have two providers available to serve your health care needs. Our primary care provider, Kallie Allender, is a nurse practitioner who sees patients of all ages. She is your partner in health as she will help diagnose, treat and prevent health issues. In addition to addressing physical issues at our clinic, we have a psychiatric nurse practitioner, Leann Lear, to work with patients to ensure that their emotional and behavioral issues are treated.
