🏀Using the "baLLL" task on FEES: When learning FEES it's a common mistake for beginners to forget about the oral phase of the swallow. We are so fixated on the pharyngeal phase and penetration/aspiration that we miss key findings. Here is a great example of additional information obtained from checking base of tongue with this simple verbal task.

This video emphasizes the importance of the right tasks on Videostroboscopy for improved diagnostics. This narrowed down the problem area which appeared to be either vocal fold nodules or a left vocal fold polyp (right side of screen) with small contact reaction on right fold. ENT confirmed vocal fold nodules on their exam. Another clue that a swelling or lesion is present is when you see mucus gathering in the same area persistently. We noticed mucus gathered in the area on the left fold pretty consistently throughout the exam as well🎯

A left vocal fold hemorrhage as seen under standard light (first clip). During the "ee" as expected we do not see movement for the exposed lateral edges of the vocal folds since standard lighting will not capture this. The next slow motion clip shows you the entirety of the obscurred left true fold once the patient is instructed to breathe in. We then turn on the strobe and are able to see mildly decreased amplitude for the right true fold. The left true fold continues to be mostly obscured due to significant compression of the left false fold; however, the lateral edge is slightly visible. We see this almost staying in place as it's visible throughout the "ee" production under strobe.

A great resource to have for anyone learning Videostroboscopy is this Laryngeal Evaluation book. The accompanying DVD features high-quality video clips of vocal fold vibration that clearly demonstrate every pathology and evaluation technique cited in the text.
