Big updates just dropped in the 2026 Guideline for the Evaluation and Management of Acute Pulmonary Embolism in Adults, developed by the American College of Cardiology and the American Heart Association Joint Committee on Clinical Practice Guidelines.

Here’s what heart failure nurses should know:

• New AHA/ACC Acute Pulmonary Embolism Clinical Categories to standardize severity classification and guide treatment decisions
• Preference for direct oral anticoagulants in eligible patients to reduce recurrent VTE and bleeding risk
• Expanded guidance on advanced therapies, including catheter-directed interventions and surgical embolectomy, for selected high-risk patients
• Formal recommendation for Pulmonary Embolism Response Teams (PERTs) to improve coordination and timeliness of care
• Emphasis on structured follow-up after acute PE to detect persistent symptoms and chronic thromboembolic disease
