Good article on ulnar collateral ligaments injuries (“Tommy John”) in throwing athletes. As this article indicates, we have increased the rate of ligament repair instead of reconstitution, in young athletes. So far we have been very happy with the outcomes with a quicker return to sports.

Osteotomies (realignment surgery) are a very valuable tool in the treatment of not only knee deformities, but also arthritis and cartilage injuries in the knee. Dr. Haro is one of the few surgeons who routinely performed osteotomies of the knee in South Carolina. For the part several years, we have been using customized patient specific instrumentation to improve the accuracy of the procedure.

Both traditionally guided osteotomies and patient-specific cutting guide osteotomies accurately corrected lower extremity malalignment. Utilization of PSCG resulted in similar procedure times and cost, but less fluoroscopy, compared to conventional osteotomy.

Graft choice matters! Over the years several studies have proposed that allograft (donor cadaver tissue) is a good graft choice in “older” patients needing an ACL reconstruction. In younger patients, numerous studies have shown they have a significantly higher retear rate than using your own tissue. Unfortunately, longer-term data now also shows that donor ligaments also fail at a higher rate in “older patients”. Why would you do a surgery that has a significantly higher rate of failure? By far away, the most common graft we have to revise is donor tissue.

Absolutely agree. While we should strive to save the meniscus if possible, however sewing together, degenerative and torn meniscus or one with non-functional tissue just doesn’t make sense. It has never made sense. There have been studies for decades showing that the repair of degenerative meniscus tears can often can lead to worse outcomes than either leaving it alone or performing a menisectomy.

Partial lateral meniscectomy shows lower risk of subsequent meniscal injury and revision ACLR at long-term, raising questions about whether preserving the meniscus at all costs is always the best approach.

Here’s a good article looking at it factors involved in the outcomes of cartilage transplantation. What the authors found is that larger grafts, older patients with more degenerative conditions, and overweight patients did not have as good of outcomes. However, overall cartilage transplants had very good outcomes long-term. In this study 73% of cartilage transplants were lasting 15 years or longer. Of the ones that do fail (15.8%) majority seem to be in the first few years (3.9).

Dr. Haro is one of the few surgeons that routinely performs osteochondral allograft transplantations for particular cartilage defects.
