Graft choice matters! Over the years several studies have proposed that allograft (donor cadaver tissue) is a good graft choice in “older” patients needing an ACL reconstruction. In younger patients, numerous studies have shown they have a significantly higher retear rate than using your own tissue. Unfortunately, longer-term data now also shows that donor ligaments also fail at a higher rate in “older patients”. Why would you do a surgery that has a significantly higher rate of failure? By far away, the most common graft we have to revise is donor tissue.

Do you utilize allograft for ACL reconstructions in your practice? This recent study of ACL reconstructions in patients older than 40 years found similar subjective outcomes but significantly greater failure rates with allograft (9 failures) vs autograft (no failures).
