Ever wonder why drug prices are so high? One of the main reasons is pharmacy benefit managers (PBMs). PBMs are middlemen that process claims between the pharmacies and the employer, government or insurance company. They mandate your copays, determine reimbursements to pharmacies and dictate what the employers pay. They charge the employers one price and reimburse the pharmacies a much lower price keeping the "spread pricing" for themselves. This last year, the 3 largest PBMs collected over $7 billion dollars. I have personally seen them attempt to overcharge patients many times. They manipulate copays to incentivize people to use their mail order pharmacies, reimbursing themselves more than other pharmacies. I have seen many pharmacy reimbursements be below the cost of the medication. This is why thousands of independent as well as non-PBM owned chain pharmacies closed this year alone.
