Thomas Family Practice was established in June of 2021. We are a full service health care practice for ages birth to end of life. Our goals is to provide optimum patient care in a secured and safe environment.

When you call the office of Thomas Family Practice you will meet Angie our office manager she will collect all personal data such as Full Name, DOB, Address, Contact Phone number, Email Address and Health Insurance information such as ID #, Provider and Contact number. We will add this to advancedMD EMR system .

Information will be collected via a verbal conversation over the phone .

Personal information / Demographics collected will be used to provide health care services and improving health outcomes through proper communication channels.
Purpose of Data Collection:
Demographic information will be used to communicate with both health insurance companies and lab companies to provide health care services.
