We try really hard to make sure that we are able to come to see you for the first postpartum home visit.

We check your vitals, weight of baby, offer and perform the Newborn Metabolic Screen, jaundice check, check baby’s oxygen levels, chat about feeding and recovery. Same things that are offered in the hospital but we come to you!

Every birth is unique. This particular birth was beautifully physiologic and dad helped catch baby. And then…..she started bleeding. A lot and FAST. People can loose a lot of blood in a very short amount of time.

When excessive bleeding happens:
1st: DONT PANIC

*Assess uterine tone and if the placenta is ready to come if it’s not already out. With an explanation of course that the rule of the uterus is that it has to be empty in order for it to stop bleeding. “Massage” helps stimulate the uterus to contract or expel clots.

*Next: “ you are bleeding a little more than usual and I would like to give you a shot of pitocin to help control that and help the placenta come. Is that ok with you?”
Then the med administration begins with communication and permission through each step.

*My protocol is to start with 10-20 units of pitocin which is an intramuscular, or IM, shot in the thigh. This is usually enough to cause the uterus to fully release the placenta which typically stops the bleeding.

*Then, we would move to an IV with TXA. Which has a different mechanism of action than the other meds which are uterotonics that cause uterine contractions.

*Assess for tearing while the IV is placed and determine if thy is the source of the bleeding.

*Next I would be prepping a 500 or 1000 ml lactated ringer IV bag with 20 units of pitocin. This helps to replace some volume and cut down on feeling so terrible after blood loss. The pitocin helps to keep the uterus contracting. The line is switched out after the TXA is completed.

*I like Methergine as it causes all of the uterine layers to contract and great at stopping bleeding but it’s best that the placenta is out before administration. It is also important to not administer to someone whose blood pressure is rising.

*last but not least is Misoprostol. It can be administered orally and rectally. It is long acting which is great to give a little peace of mind that bleeding isn’t likely to start back up when she goes home.

Not everyone bleeds too much and this is not routine for everyone. Most of the time nursing helps the uterus to contract just fine. But man I love them when we need them!

Perks of midwifery are home visit adventures! and I had a blast yesterday running around Bellingham and Oak Harbor visiting sweet postpartum families and stopping for food and beach time along the way. It’s summer and yarrow season! We stopped at Deception Pass to harvest some yarrow and collected rocks on the beach. ‘Twas a balm for the soul kind of day ✨

I am so excited to combine a few of the things that make me endlessly happy. Mothership is the new brand for my business that represents my love of space, SciFi, birth, and plants. Birth is SciFi after all! I love the blending of opposites and celebrating and honoring the magical and messy of life and love. Healing and growth, being broken down by life’s big events and then letting the process build us back up again. I get that and I am here to walk with you down these paths.
