Our Health Centers are open Monday through Friday from 8:00 am to 4:30 pm. To schedule an appointment, call 740-399-8008, email healthcenter@knoxhealth.com, or fax 740-399-8012. If you need assistance outside of our normal business hours (before 8 am or after 4:30 pm on weekdays, or on weekends), you can call 740-399-8008 and follow the prompts to access our After Hours Services. We're always here for you! Our customers are insured, under-insured, and uninsured individuals. We serve men, women, and children of all ages. Homeless individuals are also welcome.

*SLIDING FEE SCHEDULE - A sliding fee schedule at 200% above the federal poverty guidelines is offered to patients who are eligible. Minimum charge for self-pay patients is: $20 for Dental, $20 for Medical, and $10 for Counseling appointments.

If you haven't joined already, now is the perfect time! You can send messages to providers, see your results, and even schedule appointments with us right in the app!

All you have to do is download the Healow App on the App Store, sign Up and use Knox County Community Health Center's practice code: GFJBDA or search our practice "Knox County Community Health Center" in "Mount Vernon, Ohio 43050", and then access the patient portal with all of your info from visits!

Diabetes is a disease that occurs when your blood glucose, also called blood sugar, is too high. When it's not managed, diabetes can damage the eyes, kidneys, nerves, and heart, and is linked to some types of cancer.

If you have diabetes, taking charge of your health as soon as possible may help you prevent other health problems.

Tips to prevent or manage diabetes:
▪ Manage your A1C blood glucose, blood pressure, and cholesterol levels.
▪ Take care of your mental health. A mental health counselor may help you find healthy ways to cope with stress.
▪ Make lifestyle changes to slowly build healthy habits.
▪ Take your medicine consistently and on time, even if you feel healthy.
▪ Work closely with your primary care provider. They can help your prevent or manage diabetes and refer you to other health care professionals for related health problems.

Do you know about STDs and why testing is important?

If you are sexually active, being in a long-term mutually monogamous relationship with a partner who has been tested and does not have syphilis or using condoms the correct way every time are the best ways to lower your risk of contracting Syphilis.

Although Syphilis is curable with the right antibiotics from your healthcare provider, it is key to catch it early by getting tested, so it doesn't spread to others or before symptoms worsen for yourself.

Are you familiar with our Sliding Fee Scale Policy at our Health Centers?

If patients do not have health insurance, a sliding fee scale set at 200% above federal poverty guidelines is offered to all eligible patients.

A minimum charge for self-pay patients is
$20 for Dental Services
$20 for Medical Services
$10 for Counseling Services

Health Center staff can also assist patients with enrolling in health insurance or Medicaid if they qualify.
