When canals are severely calcified or blocked, we always discuss all possible outcomes with our patients—including the chance of needing apicoectomy or, in rare cases, extraction.
In this case, the canals were fully negotiated, and the treatment was completed successfully after extensive work with hand files and rotary files. The tooth needs a new crown.

This patient presented with severe pain from the cracked #30. It was very difficult to anesthetize this "hot tooth." After seven cartridges of IAB, LB, intra-ligamental, and intra-pulpal, RCT was completed. The patient felt better after the treatment. The tooth had five canals - two distal and three mesial canals.
