We are an Indiana owned skilled nursing facility in Muncie, Indiana. Our motto is “Hoosiers Helping Hoosiers”

We specialize in behavioral and dementia care as well as rehab to home and long term care patients. We also offer respite and hospice stays.
We are open 24 hours a day 365 days a year and accept weekend and holiday referrals as well as admissions.

Not sure how to pay for a stay at our facility? We can help. Our financial team can assist with Medicaid applications and updates. We accept Medicare, Medicaid, private pay, VA, and Anthem, and Humana Managed Medicare Plans.
