This helps ensure we can treat another patient who may need that spot. While we prefer 48 hours notice, our team appreciates any efforts to reach out to us to let us know you're unable to keep your appointment.

He refused treatment—until they called me.

This patient had been in an academic center system for 6 weeks—seen, tested, managed—but increasingly uncertain. Before his next step, he gave the team my direct number and said: “Please call my primary care physician. I’m not moving forward without her.”

He had been doing well with an advanced pacemaker. But somewhere along the way, he was labeled with heart failure and started on a blood thinner.

Individually, those decisions weren’t irrational. Together, they didn’t make sense.

What was missed wasn’t knowledge. It was context. When connected, the answer was clear: pacemaker-induced cardiomyopathy, not primary heart failure.

The course corrected quickly, but only because the patient paused the system long enough to have his primary care team reconnect the dots.

The greatest risk in healthcare today is not what we don’t know. It’s what we fail to connect.

Regular physical activity helps:

How will you take time out to rejuvenate today?

Great post from Dr. Joanne Niere-Ramos, pictured here with her husband, Dr. Jonathan Ramos, and their family.

The Fridays we work Saturdays have kind of become our thing.

We usually have half day clinic, then go straight to picking the boys up from school. From there, it’s an early dinner at Coppolillo’s—our spot.

We always ask them the same question: what did you love about school this week? And somehow those conversations end up being the best part. It’s our way of slowing things down and really hearing them.

After that, it’s really about spending time together—hanging out, being present, or learning about Pokémon cards, trying to soak it all in because they’re growing up way too fast.

And at some point, I’m usually in the backyard with JJ, just playing and unwinding for a bit.

Balancing work and motherhood isn’t easy, but it’s everything. And these small, intentional moments? They’re the ones that make it all feel whole.

A recent Bloomberg News headline caught my attention: “Risk-taking is back.”

Innovation, especially in healthcare, requires courage — now more than ever. Innovation is not built on comfort. It is built on the willingness to step into uncertainty.

It took me down memory lane — 25 years ago.

As a young internist with no formal training in business operations or P&L, I stepped into building a different kind of primary care model.

There were no dashboards. No automation. No . Just paper charts, long phone calls, and a commitment to doing what was right for patients.

Even then, the principles were clear: risk stratification, timely cross-disciplinary collaboration & early intervention.

We were building population health before we had the language for it. Not scalable, but directionally right. And it came with risk.

While many chose stability, some of us chose to build — without a roadmap — because we believed care could be different.

Today, we are at a similar inflection point. But now we have something we didn’t before: technology & intelligent automation.

We are also seeing a meaningful shift in the next generation — one that is increasingly drawn to purpose, willing to take risks, and motivated to build, rather than simply choosing the safety of established paths.

For those who see the opportunity to build what actually needs, this is the moment to lean in.
