Spring Creek Post-Acute Rehabilitation offers a variety of services to our residents. We are a 226-bed long-term care, Medicare-certified facility which provides comprehensive rehabilitation and restorative services. Each room is cable and phone ready. We love our community and want to use social media to connect with you! I know that this time of the year is very hectic for everyone and to see everyone come together and to be a blessing for our residents at this time is so amazing!! We appreciate each and everyone of you for making another year so special for our residents ❤️ And thank you to our activity department for making another year with the Angel Tree program a success Jaleesa, Missy, Boni, and Ericka you guys rock!!!

Spring Creek Health Care offers a variety of services to our residents. We are a 226-bed long-term care, Medicare-certified facility which provides comprehensive rehabilitation and restorative services. We offer private and semi-private rooms. Each room is cable and phone ready.

Our success rate with our short term rehabilitation program has been extraordinary. The comprehensive care plans are designed to facilitate a quick and complete recovery. Plans are incorporated into the individual resident’s rehabilitation programs with the goal of their independence and the ability to return to community living. Spring Creek Health Care also emphasizes support and education for residents and their caregivers prior to returning home. We love our community and want to use social media to connect with you!
