To start the process with me for your medical cannabis provider certificate, CLICK on the link below to request an appointment.

You will then be emailed the legal clinical paperwork to electronically sign along with health questionnaire, PTSD self screener, and downloadable educational information. These forms must be signed and filled out 24 hours prior to your scheduled visit. 

Provider visits for medical cannabis certificates are not covered under any insurance because cannabis is federally illegal. Medicare and Medicaid are considered federal programs

My visit cost is $175 (+3% credit card fee). You will have a $25 application fee to the Kentucky cannabis program.

Here’s a quick reference guide about the process for qualified patients. Knowledge is power, so I do recommend if you get your medical cannabis card you learn everything you can about the program.
As your medical cannabis provider, I will guide and educate you about the process at your visit. But it is always best to get information straight from Kentucky’s program. 

“Disqualifying felony offense" means:
(a) A felony offense that resulted in the person being classified by the Department of Corrections as a violent offender under KRS 439.3401;
(b) A violation of a state or federal controlled substance law that was classified as a felony in the jurisdiction where the person was convicted, except:
1. An offense for which the sentence, including any term of probation, incarceration, or supervised release, was completed five (5) or more years earlier; or
2. An offense that, as determined by the cabinet, consisted of conduct for which this chapter would likely have prevented a conviction, but the conduct either occurred prior to the enactment of this chapter or was prosecuted by an authority other than the Commonwealth of Kentucky

Senate Bill 47 was signed into law in Kentucky legalizing medical cannabis for Kentucky residents beginning January 1, 2025.

The first step to become a registered cannabis card holder in Kentucky is by creating a bona fide provider/patient relationship with a licensed medical cannabis provider in Kentucky. This would be accomplished by your first visit with me. These visits are not covered by any insurance companies. The cost of your initial visit is $175.

1. Qualification criteria for Kentuckians to become a registered cannabis cardholder

• Be a Kentucky resident
• Have a diagnosed qualifying medical condition
• Have no disqualifying felony offense convictions

2. How to become a registered card holder in Kentucky

• Visit a medical cannabis provider to obtain a written certification
• Submit an application to the Kentucky cabinet 
• Receive an application decision from the Kentucky cabinet and download a registry ID card

3. Kentucky Cabinet medical cannabis application requirements
• Cardholder’s information
• Written certification from medical practitioner
• Ky Application fee of $25
• Notarized page with applicant’s signature 

I highly recommend you go to the Kentucky office of medical cannabis website to review the laws and regulations for yourself.
