TICK-BORNE DISEASE & PREVENTION in KIDS

As some of you may know, May was Lyme Disease Awareness Month.
Outdoor activities have been encouraged throughout the pandemic which makes it a good time to review tick borne disease. Remember where there is nature, there are ticks, and where there are ticks, there is Lyme (Borrelia burgdorferi) and co-infections.

Two families, Ixodes scapularis (hard bodied ticks) and Argasidae (soft bodied ticks) are responsible for transmitting most diseases to humans in the US. Once a tick is infected with a pathogen it usually survives and transmits it to its host. Humans are usually incidental hosts. Ticks effectively transmit bacteria, protozoa, rickettsia, nematodes, and toxins to humans during feeding when the site is exposed to infected salivary gland secretions or regurgitated mid-gut contents.

Three species of ticks are responsible for most human disease:
Ixodes scapularis (Blacklegged tick), Amblyomma americanum (Lone Star tick), and Dermacentor variabilis (American dog tick). Each is capable of transmitting multiple pathogens since ticks have no boundaries.

Tick activity is greatest between April and September, but here in California, tick exposure can occur all year round. A CDC report in 2019 showed that tick borne and mosquito-borne infections have tripled over the last 13 years. This includes Lyme disease, which accounts for 82% of all tick-borne diseases.

The CDC now estimates that 476,000 people in the US each year are diagnosed and treated for Lyme disease. Children ages 0-19 years represent 29% of reported Lyme cases from 2001-2017. After a course of treatment, 15-20% or more of patients will have long term symptoms. Many are unaware of the devastating physical, mental, and emotional toll that Lyme disease has.
In young children, it may be difficult to assess symptoms so it is important to note change in behavior, energy level, appetite, lack of interest in activities they usually love to do, as well as GI symptoms.
Other manifestations including cognitive impairment, mental confusion, memory loss, headaches, difficulty concentrating, speech difficulty, visual and hearing problems, dizziness, mood swings, outbursts, depression, sleep disturbance, OCD, and seizure activity, all of which affect their educational experience.
With summer fast approaching and children spending more time outdoors, it is important for everyone to learn about and take precautions against ticks and the diseases they carry.

Prevention is the only way to end the TBD epidemic. Education is your best defense against Lyme and tick-borne diseases.

Create a tick-safe zone at home:
Remove leaf litter, clear tall brush and grass around your home, and mow your lawn frequently.
Keep playground equipment, decks, and patios away from yard edges and trees.
If you live near a wooded area, place a 3 ft wide barrier of gravel or wood chips between the areas.
Put up a fence to keep unwanted animals out.
Keep your yard free of potential hiding place for ticks (eg. furniture, mattresses)
Stack wood neatly and in a dry area.

Outdoors safety tips:
Wear a hat, light clothing, and long pants tucked into your hiking boots or socks
Avoid DEET due to its toxicity!
Apply tick repellent on your skin (Bite blocker, Repel lemon eucalyptus, Badger anti-bug balm/spray, Avon skin so soft) and 0.5% permethrin on clothing to repel mosquitos and ticks.
Walk on trails and avoid sitting on logs or leaning against trees.
Do daily body checks for ticks after coming indoors. Just because a tick carries Borrelia and bites you does NOT mean that you will get Lyme disease.
Always check your pets for ticks when they come in from being outdoors.

Remember, the goal is to PREVENT Lyme disease and co-infections.
Lyme disease, if caught early and treated appropriately does NOT have to go on to be a devastating chronic illness.

So what happens if your child gets a tick bite?

The following 6 steps are summarized from Dr. Alexis Chesney’s book, “Preventing Lyme and Other Tick-Borne Diseases.”

Step 1: Immediately Remove and Identify the Tick
Use the O’Tom Tick Twister to remove the tick, identify the tick, and put it in the plastic bag.

Step 2: Apply First Aid
-After you remove the tick, place 2 drops of Andrographis tincture on the tick bite wound.
-Take homeopathic Ledum palustre 30c after a tick bite to encourage healing. (3 pellets under your tongue 3x daily x 3 days)
-Apis mellifica 30c may be added if the tick bite causes a rash or localized swelling which feels hot to the touch, and relieved by cold compresses. Dosage is the same as Ledum palustre.
-After a tick bite, please report the rash to a medical professional.
It is very important to acknowledge that no rash does not mean no disease!

Step 3: Have the Tick Tested for Pathogens
Send the tick to www.tickreport.com or www.ticknology.org for testing.
The turnaround time for both of these labs is around 3 days from receipt of the tick.

Step 4: Begin the Herbal Prophylactic Protocol
For adults, take 1 teaspoon of the Deer Tick Bite Formula (DTBF) in water 3 times a day,
30 minutes before breakfast, lunch, and dinner, for 30 days.
While it is safe to use DTBF with children, the extremely bitter taste is often a deterrent. There are usually more palatable herbal formulas available, some in glycerin base.

Step 5: Watch for Symptoms
Immediately report signs or symptoms of tick-borne diseases to a medical professional.

Step 6: Get Tested (Maybe)
Consult with a medical professional.

Are antibiotics warranted?
Most Lyme-literate practitioners believe that a single dose of doxycycline has never been effective in preventing Lyme disease.

"Based on animal studies, the International Lyme and Associated Diseases Society (ILADS) recommends that known blacklegged tick bites be treated with 20 days of doxycycline (barring any contraindications)."

At least a 3-week course of antibiotics is needed to ensure that you kill Borrelia in all of its life forms (cell wall or spirochete form, cystic form or cell wall-deficient form, intracellular form, & biofilm persister form). By stopping antibiotics too early, you may not kill the Borrelia that is hiding out in its dormant cyst form and waiting for just the right moment to reactivate and convert to its free-floating spirochete form to cause trouble at a future date.

The antibiotic chosen may be herbal, pharmaceutical, or a combination of both.
For adults, a 3-week course of doxycycline may be recommended. The CDC now recommends “doxycycline as the treatment of choice for children all ages with suspected tick-borne rickettsial disease… Previous concerns about tooth staining in children less than 8 years stem from older tetracycline-class drugs that bind more readily to calcium than newer members of the drug class, such as doxycycline.”
Consider other alternative antibiotics for children such as amoxicillin, azithromycin, or cefuroxime axetil if photosensitivity develops with doxycycline.
Be aware that chronic use of antibiotics can cause fungal overgrowth impacting their gut microbiome.

Herbal anti-microbials targeted to Lyme and co-infections can also be used alone or in combination with pharmaceutical antibiotics. I prefer using herbal anti-microbials due to persister forms & dna transformation of lyme disease that could promote antibiotic resistance. There is also less relapse when using herbs.

Stephen Buhner, a highly regarded Lyme herbalist, recommends the herb Astragalus to boost one’s immune system and prevent Lyme transmission.
After a new tick bite, he recommends the adult dosage of 3000mg daily for 30 days.
For prevention, if you live in a highly endemic Lyme area and don’t want to be on antibiotics for the entire season, Buhner recommends that adults take Astragalus 1000mg daily.
For a 1 year old, you can add 10 drops of the tincture to their diet three times a day. Older children can be given larger doses, around 1/8 tsp, mixed in food daily.
Immediately after a tick bite, Buhner has also found applying a topical paste made of Andrographis tincture and green clay to be helpful in preventing infection.

If you or your child gets bitten by a tick, please discuss what anti-microbial regimen is best with your physician or a Lyme-literate practitioner. (ilads.org)

Lyme Disease Association (LDA) has a pamphlet written especially for parents and educators called the ABC’s of Lyme Disease.
This pamphlet contains information from experts on the impact of Lyme disease on children’s education, and hundreds of thousands have been distributed.

The LDA also has Tickles, a prevention video, in the “Lyme, Kids and Schools” website section.

National Capital Lyme Disease Association (NatCapLyme) also has an educational and awareness outreach campaign for children and adults called Tick Busters: Wanted Dead or Alive (tickbusters.org). This year their focus is on an awareness and prevention educational program targeting children since they are the most vulnerable population given they play outdoors extensively.

How One Little Bug Bite Can Change Your Life

Cathy, a young, healthy, and active adolescent, had an array of chronic symptoms for about 6 months: fatigue and shortness of breath, memory issues and concentration problems, headaches, light sensitivity, joint pain, cough, appetite swings, gas & bloating due to certain foods, red eyes, blurred vision, excessive sweating or night sweats and mood swings. About this time, she also began experiencing vibratory sensations which gradually became constant. She described these as a continuous internal vibration (like a bee buzzing). It started in her arms, moved to her legs, and eventually her entire body. It was so severe that she was unable to sleep due to the continuous vibrations.

Cathy’s parents took her to over 10 doctors. She had blood work, cat scans, spinal taps, EKG’s, EEG’s, and an upper and lower endoscopy. No one was able to find the root cause of the problem. She was evaluated by a neurologist who initially suspected possible demyelinating disease (Multiple Sclerosis) and recommended an MRI which turned out to be negative.

Finally, another doctor did more tests and took the time to see what the other doctors had missed. Cathy was diagnosed with Lyme disease. This doctor put her on 30 days of antibiotics and said she would be back to normal. Unfortunately, when the 30 days of antibiotics were over, she started to nose dive again.

Cathy came to us with all the symptoms of late-stage Lyme disease. Through various in-depth testing, we discovered she had two common co-infections of Lyme disease, Bartonellosis and Babesiosis. Through comprehensive testing, we found she suffered from immune dysfunction, inflammation, mitochondrial dysfunction, various nutritional and enzyme deficiencies, toxicities (heavy metals, mold), and endocrine abnormalities.

Over the course of 2 yrs we established an individualized protocol with the aim of addressing infection, lowering inflammation, and restoring immune balance.
Today Cathy lives as a young and vibrant teenager once again.

PANS/PANDAS: Finding the Underlying Cause of Sudden Behavior Changes in Children

Nine year old Erica, always an A-student who rarely got into trouble, was now having difficulty focusing and paying attention. Being a sensitive child, she displayed problems with social interaction and sensory issues related to texture. In January 2015, Erica’s behavior suddenly turned ugly, and she became a rebel in school.
She tormented a boy to the point of physical violence. Rocking & shaking her hands became more dramatic, and her rage attacks became more frequent and scary. Going to restaurants almost always set her off, while previously she had no problems. The school principal recommended switching her to a charter school which might better fit her needs.

Erica’s mother brought her to several specialists including a neurologist and psychologist who diagnosed Erica with Autism Spectrum Disorder, but offered no answers. Her primary doctor then referred Erica to a psychiatrist who diagnosed her with anxiety and depression, and recommended a trial of anti-psychotics. Her mother declined pharmaceuticals, and instead opted for weekly counseling.

This is a story we hear on a regular basis from misdiagnosis to unsuccessful treatment or lack of a treatment, all adversely affecting the child, the entire family, loved ones, and classmates.

In April, 2017, Erica’s mother sought us out. With testing, we uncovered infectious triggers that included streptococcus, lyme, and bartonella, a co-infection of lyme, which together with her symptoms indicated a diagnosis of PANS. Later on we also uncovered that she had an underlying mold toxicity. Not a quick fix, but Erica’s mother agreed to both conventional and alternative therapies that would gradually and gently help her daughter get better.

Erica’s course of treatment over the next two and a half years was aimed at safely eliminating the causes of her aberrant behavior rather than covering up her symptoms. This involved several rounds of antibiotics including anti-fungals and IV ozone to eradicate the infections. We also gave her foods that were healing to nourish her, addressed her unhealthy gut microbiome, corrected any vitamin/mineral imbalances, and boosted her immune system using various herbal and nutritional supplements.

To the delight of her family, friends, and teachers, Erica has made a full recovery, excelling in school, and is back to her baseline health as of October, 2019.
